| Objective:The aim of this study was to assess the correlation between hormone profile and semen parameters in men volunteers.We tried to find quantitative parameters to diagnosis or assess the sperm parameters,sperm acrosome reaction,male fertility.Methods:A total of 116 male volunteers were recruited in our study.Serum level of follicle-stimulating hormone(FSH),luteinizing hormone(LH),testosterone(T),estradiol(E2),prolactin(PRL),and the combinative ratios of these hormones,were compared individually with sperm parameters,sperm acrosome reaction and fertility index by Pearson analysis or Spearman analysis.The sperm parameters included sperm concentration,total sperm count,percent motile sperm count,percent normal sperm count,normal sperm morphology,sperm acrosome reaction,sperm concentration,sperm volume.We used multiple linear regression to explore the combining hormonal combinative ratios with sperm acrosome reaction and male fertility.We used the ROC curve to find the value to evaluate the different kinds of ratios to diagnosis the normal serum AR and normal fertility index.After the ROC analysis,we used the students t test to compare the sperm parameters according to the two groups.Results:After Pearson correlation or Spearman correlation,we found that he ratio of testosterone to luteinizing hormone(T/LH)(r=0.223,p=0.016)was positively correlated with sperm count(p<0.05),the serum T/FSH of 116 male volunteers had a statistically positive correlation with sperm count(r=0.248,p=0.007),a statistically positive correlation with sperm concentration(r=0.238,p=0.010),a statistically negative correlation with PR(r=-0.183,p=0.049),a statistically positive correlation with the ratio of testosterone to prolactin(T/PRL),and a statistically positive correlation with sperm response rate(r=0.187,p=0.045),FSH/LH had a statistically positive correlation with semen volume(r=0.194,p=0.037),FSH/PRL had a statistically positive correlation with sperm volume(r=0.210,p=0.037).There was a statistically positive correlation between prolactin and normal sperm shape(r=0.233,p=0.012)(p<0.05),a statistically positive correlation between testosterone and sperm concentration(r=0.215,p=0.021),and a statistically positive correlation between sperm count(r=0.241,p=0.009)(p<0.05).(2)Multiple linear regression analysis was conducted on 116 male volunteers sperm AR and male fertility index and related factors,and it was found that FSH/PRL was linearly correlated with sperm acrosome response rate,with partial regression coefficient of 32.684,p=0.014,and FSH/E2 was linearly correlated with sperm AR,with partial regression coefficient of-369.909,p=0.039,In the multivariate linear regression analysis of the fertility index and various factors,it was found that FSH/PRL value was linearly correlated with the fertility index,and the partial regression coefficient was-14.514,p=0.020.It was also found that there was no linear correlation between sperm defects and sperm acrosome response rate(p>0.05),while there was linear correlation between sperm head defects and fertility index(p<0.01),and the partial regression coefficient was-0.872,p=0.000.(3)The ratio of FSH/PRL predicted the normal cut-off value of sperm AR was 0.349,the corresponding area under the curve and 95%CI were 0.633(0.513-0.754),the sensitivity was 58.8%,the specificity was 70.7%,the serum prolactin concentration was 7.470,the area under the curve and 95%CI was 0.694(0.558-0.830),and The ratio of T/LH predicted the normal cut-off value of sperm morphology was 1.325,area under the curve and 95%CI:0.475(0.366-0.583).Conclusion:(1)Sex hormones,gonadotropin and their ratio were correlated with sperm count,sperm concentration,sperm motility,sperm normal morphology rate and acrosome response rate.(2)With the increase of the serum levels of follicle-stimulating hormone and prolactin ratio,then the sperm acrosome reaction rate decreases.The increase of follicle-stimulating hormone and estrogen ratio make the sperm acrosome reaction rate increase.The follicle-stimulating hormone and prolactin ratio increase lead to rising fertility index decreases,sperm head defects also lead to lower fertility index.(3)Follicle-stimulating hormone and prolactin ratio of 0.349 was the best cutoff value to predict normal sperm acrosome reaction,serum PRL value of 7.470 was the best cutoff value to predict the rate of normal sperm morphology. |
