Objectives To analyze the clinical characteristics of sex hormones,especially estrogen levels in depression patients of different ages and genders.Study the sensitivity and specificity of estrogen changes in patients with depression,and to provide reference for early clinical screening and biomarkers in patients with depression.Methods A total of 100 outpatients and inpatients with depression diagnosed in the Department of psychiatry of Kailuan Mental Health center affiliated to North China University of Technology were selected as the case group.?There are 40 male patients and 60 female patients,according to age,they are divided into young male group?16-45 years old?,middle-aged and old male group?greater than or equal to 45 years old?,young female group?16-45 years old?,perimenopausal female group?45-55 years old?and old female group?greater than 55 years old??.100 healthy volunteers were set as a control group.Hamilton Depression Scale?HAMD-24 items?was used to evaluate the two groups.The levels of sex hormones in serum of all the participants were measured by magnetic particle chemiluminescence and autolumo A200 chemiluminescence detector,compare the levels of sex hormones in each group,analyze whether sex hormones are risk factors for depression patients,and study the sensitivity and specificity of estradiol in depression patients.Results 1 There was significant difference in the level of female sex hormone between different age groups in the control group?P<0.017?.2 There was significant difference in the level of female sex hormone between different age groups in the case group?P<0.017?.3 There was a significant difference in the level of sex hormone among men of different ages in the control group?P<0.05?.4 There were significant differences in the levels of sex hormones among men of different ages in the case group?P<0.05?.5 The level of luteinizing hormone?LH?in young women with depression was lower than that in healthy young women?Z=-2.191,P=0.028?,in the binary Logistic regression analysis of hormone level and the risk of young depression patients,low E2was the protective factor of young female depression patients?OR=0.960,95%CI 0.928-0.993,P=0.019?,The area under ROC curve of estrogen in the diagnosis of depression in young women was 0.615?95%CI 0.428-0.797,P=0.224?,when the estrogen threshold was 98.71 pg/mL,the sensitivity was 40.0%,and the specificity was 100.0%.6 There was significant difference in the level of sex hormone between the perimenopausal women and the control group?P<0.05?,in the binary logistic regression analysis of hormone level and the risk of depression in perimenopausal women,low E2may be the risk factors of depression in perimenopausal women?OR=1.024,95%CI 0.999-1.048,P=0.057?.7 There was significant difference in sex hormone level between the case group and the control group?P<0.05?,in the binary Logistic regression analysis of hormone level and the risk of depression in elderly women,low FSH may be a protective factor in elderly women with depression?OR=0.851,95%CI 0.768-0.943,P=0.002?.8 There was a significant difference in the level of sex hormone between the case group and the control group?P<0.05?,in the binary Logistic regression analysis of hormone level and the risk of depression in young men,low FSH may be the protective factor for depression in young men?OR=0.748,95%CI 0.593-0.945,P=0.015?,low luteinizing hormone?LH?may be a risk factor for depression in young men?OR= 1.443,95% CI 1.047-1.989,P=0.025?.9 There was significant difference in the level of sex hormone between the case group and the control group?P<0.05?,the area under ROC curve of estrogen in the diagnosis of depression in middle-aged and old men was 0.769?95% CI 0.621-0.916,P=0.004?,When the estrogen threshold was 56.52 pg/mL,the sensitivity and specificity of the diagnosis were 0.0% and 100.0% respectively.Conclusions The detection of sex hormone level is affected by age,gender and other physiological factors.There is abnormal sex hormone level in depression patients,and the change of sex hormone level in depression patients with different age and gender is different.Sex hormones,especially estradiol?E2?,have a certain predictive value for depression,and can provide a reference for clinical diagnosis of depression.Figure 2;Table 16;Reference 136...
