Objectives To explore the status and influencing factors of smoking relapse of male patients with coronary heart disease in 1 year after percutaneous coronary intervention(PCI),so as to provide theoretical basis for clinical smoking cessation nursing intervention and health consultation.Methods From June to October 2019,We conduct a face-to-face questionnaire survey in male patients with coronary heart disease and one year after PCI who smoked before PCI but quit smoking during the period of interventional therapy in the department of Cardiology of a third-grade a hospital in Tangshan city were selected by the method of convenience sampling.The questionnaire included general information scale,smoking cessation education and smoking cessation related actions designed by researcher,physical health level,fagerstorm test for nicotine dependence,awareness of smoking hazard,general anxiety disorder(GAD-7),patient health questionnaire(PHQ-9),partner interaction questionnaire(PIQ).The SPSS22.0 statistical software was used for statistical analysis,univariate analysis was performed by chi-square test,and multivariate analysis was performed by binary Logistic regression analysis.Results 1 A total of 290 patients were investigated in this study.150 patients had relapse within 1 year after PCI,the relapse rate was 51.7%,and 140 patients did not relapse,accounting for 48.3%.2 77 patients(26.6%)relapsed within 3 months after PCI,33 patients(11.3%)relapsed within 3 ? 6 months after PCI,23 patients(7.9%)relapsed within 6 ? 9 months after PCI,17 patients(5.9%)relapsed within 9 ? 12 months after PCI.The chi-square trend test of relapse rate and time show that the P < 0.001,relapse rate and time increasing linear relationship,the 3 months after PCI is the peak of relapse.3 The univariate analysis showed that social demographic factors in different age,working condition of patients relapse rate difference was statistically significant(P <0.05)and clinical factors whether patients with acute onset of disease relapse rate difference was statistically significant(P < 0.05),smoking related information factors have different smoked,quit smoking experience,different tobacco dependence degree and the different awareness of smoking hazard patients relapse rate difference was statistically significant(P < 0.05),education factors of doctors and nurses in clinical doctors and nursers to quit smoking education and have to quit smoking education are satisfied patients who relapse rate difference was statistically significant(P < 0.05),the body health factors in different state of angina pectoris patients relapse rate difference was statistically significant(P < 0.05),psychological factors in the presence of anxiety depression,changes of smoking abstinence self-efficacy of patients relapse rate difference was statistically significant(P < 0.05),social support factors in the presence of the emotion and instrumental support,with and without complaining about smoking and criticism of smokers patients relapse rate difference was statistically significant(P <0.05),Among the smoking cessation related action factors,there were statistically significant differences in relapse rates among patients who remove all tobacco products from the house,do some things else when you crave a cigarette,and quit immediately without cutting back first(P<0.05).The relapse rate of patients with age ? 65 years old,working,acute onset,smoked > 30 years,has a smoking cessation experience,severe tobacco dependence,low awareness of smoking hazard,doctors and nurses did not provide smoking cessation education,are not satisfied with smoking cessation education,stable angina pectoris,anxiety,depression,low smoking abstinence self-efficacy,quittingwithout emotion and instrumental support,complaint and criticism from smoking problem,not to remove all tobacco products from the house,have no idea when want to smoking,considered by reducing smoking to stop smoking is higher.4 Multivariate analysis showed the working condition(working: OR=2.713,95%CI=1.230?5.986),degree of tobacco dependence(severe:OR=3.487,95%CI=1.228?9.904),awareness of smoking hazard(high: OR=0.341,95%CI=0.161?0.491),smoking cessation education of nurses(educated: OR=0.051,95%CI=0.019?0.137),angina status(stable: OR=5.052,95%CI=1.791?14.250),depression(depression:OR=3.487,95%CI=1.228?9.904),smoking abstinence self-efficacy(high: OR=0.181,95%CI=0.067?0.491),emotional support(OR=0.253,95%CI=0.115?0.558),instrumental support(OR=0.411,95%CI=0.191?0.883),complaints about smoking(OR=7.835,95%CI=3.492?17.580),critical of smokers(OR=4.074,95%CI=1.716?9.671),remove all tobacco products from the house(OR=0.458,95%CI=0.213?0.984),and quit immediately without cutting back first(OR=0.204,95%CI=0.093?0.446)were influential factors for relapse.Conclusions 1 The male patinents of coronary heart disease within 1 year after PCI was 51.7%,and the 3 months after PCI is the peak of relapse.2 working condition,tobacco dependence,awareness of smoking hazard,nurse education,angina state,depression,smoking abstinence self-efficacy,emotional support,instrumental support,complaints about smoking,critical of smokers,remove all tobacco products from the house and quit immediately without cutting back first are influencing factors for tobacco relapse in male patients with coronary heart disease after PCI.Working,severe tobacco dependence,stable angina status,depression,being complained and criticized for smoking are risk factors for tobacco relapse in patients with coronary heart disease after PCI.Higher awareness of the smoking hazard,nurses providing smoking cessation education,high self-efficacy of smoking cessation,emotional and instrumental support,remove all tobacco products from the house,quit immediately without cutting back first are protective factors for tobacco relapse in patients with coronary heart disease after PCI.Figure1;Table13;Reference 169...
