Background: Functional gastrointestinal disease(FGID)is also called disorders of gut-brain interaction.Its symptoms are mainly related to dynamic disorder,visceral hypersensitivity,changes in mucosal and immune functions,changes in intestinal flora,and abnormalities in central nervous system processing functions.Abdominal pain is a common symptom of functional gastrointestinal disease(FGID),which is an important factor for patients to see a doctor and affect their quality of life.Central mediated abdominal pain syndrome(CAPS)is one of the typical chronic abdominal pain diseases in FGID.It was also known as "functional abdominal pain syndrome","chronic idiopathic abdominal pain" and "chronic functional abdominal pain".Patients with CAPS occupy a lot of medical resources and obviously increase the economic burden.There are many studies on abdominal pain,especially in irritable bowel syndrome(IBS),but few focus on CAPS.Objective: To summarize the pathogenesis,clinical manifestations,differential diagnosis and treatment of CAPS with clinical case and related literature at home and abroad.To strengthen clinicians' understanding of CAPS,so as to provide reference for clinicians in future diagnosis and treatment.Methods: Three detailed clinical datas of CAPS in the Department of Gastroenterology of Hebei General hospital were reported.We search the Wanfang Database,CNKI,CBM,FMRS and Pubmed,with the keywords of“centrally mediated abdominal pain syndrome ? functional abdominal pain syndrome?chronic idiopathic abdominal pain?chronic functional abdominal pain”,and summarize relevant literatures and case reports.Conclusion: CAPS is a FGID with abdominal pain as the major performance,which is characterized by severe,diverse,extensive and poorlylocalized abdominal pain.The main pathogenesis of CAPS is the abnormal function of central pain regulation pathway and the formation of central sensitization.Mental comorbidity also plays an important role in its pathogenesis.Therefore,the establishment of good doctor-patient communication,in-depth understanding of the psychosocial factors of patients with CAPS,combined with their clinical manifestations and relevant negative auxiliary examination results,can make a clear diagnosis.Through the multidimensional clinical profile of the overall situation of the patient,to provide patients with a variety of treatment options,and at the appropriate time for referral,can be just receive the best therapeutic effect.
