Objective: To analyze the clinical characteristics,risk factors and related factors of poor outcomes of patients with acute cerebral infarction with extremely severe posterior circulation stenosis or occlusion,and provide more accurate prevention and treatment for such patients.Methods: The clinical data of patients with acute cerebral infarction with extremely severe posterior circulation stenosis or occlusion(Posterior circulation vascular stenosis?70% or occlusion.The number of vascular stenosis is 5,which include bilateral vertebral artery,basilar artery,bilateral posterior cerebral artery.)were collected,and the clinical data of patients were analyzed retrospectively and their correlation with disease prognosis.Results: 1.Demographic characteristics: Men(74.3%)are more common than women in this study,with more than half of the middle-aged patients(56.8%);hypertension(79.7%)is the most common risk factor;2.Clinical symptoms and signs: Paralysis(77.0%)was the most common symptoms and signs;3.Imaging features: The infarct site was most common in the mixed group(56.8%),especially the pontine infarction(73.0%);4.Complications: Lung infections(37.8%)were the most common;5.Clinical outcome: Nearly half(47.3%)of patients had a good outcome;6.Analysis of related factors: diabetes(P=0.042;OR=3.471,95%CI=1.043-11.549)and complications(P<0.001;OR=7.607,95%CI=2.589-22.355)were independent predictors of poor prognosis.Conclusions:1.The most common risk factor of patients with acute cerebral infarction with extremely severe posterior circulation stenosis or occlusion was hypertension;Mixed group was the most common type,especially pontine infarction;the most common clinical manifestation was paralysis.2.Diabetes and complications were independent predictors of poor outcomes in patients with acute cerebral infarction with extremely severe posterior circulation stenosis or occlusion.
