Objective:1. To explore whether passive smoking intervention can affect the healing process of third and fourth phase pressure ulcers in rats.2.To study the effect of passive smoking intervention on the exp ression of MMP-9 and TIMP-1 on wound surface during the healing process of third and fourth phase pressure ulcers in rats.3.To explore the relationship between the changing levels of MM P-9,TIMP-1 and MMP-9/TIMP-1 ratios and the healing of pressure ul cers in rats,so as to provide a new therapeutic target for promoting t he healing of pressure ulcers.4.To study the expression level of MMP-9 and TIMP-1 on woun d surface of third and fourth phase pressure ulcers in rats,and its rel ationship with the damage degree of pressure ulcers.5.To provide theoretical basis for formulating nursing measures in the future and guiding clinical comprehensive assessment of pressure ulcer risk.Methods:1.Animal groups:sixty healthy male SD rats adaptation 1 week,t hen according to the size of weight number by random number table method randomly divided the rats into smoking stage?pressure ulcer s group,smoking stage?pressure ulcers group,non smoking stage?ulcers group,non smoking stage?pressure ulcers group,15 in ea ch group,body quality no statistical difference?P>0.05?.2. Rat smoking intervention:according to Liu,X et al.[1]method of preparing the rat model of passive smoking,the rats were put into a closed smoking device.The rats were given 10 cigarettes each time?lasti ng 1 hour?,2 times a day,2 times an interval of 6 hours,5 days a w eek,for a total of 12 weeks of continuous smoking.Control rats were put into the same rat box,but no smoking intervention was conducted.3.Building third and fourth phase pressure ulcers in rats:smoking stage?pressure ulcers group and smoking stage?pressure ulcers group rats after 12 weeks of smoking intervention,four groups rats,wi th reference to Peirce Wassermann[2-3]method to build pressure ulcers m odel in rats.Model judgment standard reference Huang Qian[4]study.4.Result evaluationThe pathological changes of wound surface after the formation of pressure sores,wound surface area,MMP-9,TIMP-1 protein expression level and MMP-9/TIMP-1 ratio in the healing process were observed.Results:1.Comparison of pathological changes of pressure sore wound in rats1)Stage?,?pressure ulcers wound tissue pathology change,b ut?pressure ulcers wound pathological damage is more serious.2)Both the smoking group and the non-smoking group had patho logical changes in the wound tissue of pressure sores,but the patholo gical damage of the smoking group was more serious than that of the non-smoking group.2.Comparison of wound area of pressure ulcers in rats1)Rats stage?pressure ulcers wound area in pressure ulcer heal ing process of 1st,3rd,7th and 14th were significantly less than stage?pressure ulcers,differences were statistically significant?P<0.05?.2)The wound area of the pressure ulcers in the smoking group w as significantly larger than that in the non-smoking group on the 1st,3rd,7th and 14th day of the healing process of the pressure ulcers?P<0.05?.3. Immunohistochemical test results of MMP-9 in the wound tissu e of pressure ulcers1)In pressure ulcer healing process of day 1st,day 3rd,day 7th and day 14th,?phase of pressure ulcers in the organization and a mount of protein expression of MMP-9 was obviously higher than tha t of?phase of pressure ulcers in the organization,the difference h ad statistical significance?P<0.05?.2)On day 1,day 3,day 7 and day 14 of the pressure sore heal ing process,MMP-9 protein expression in pressure sore tissues of the smoking group was significantly higher than that of the non-smoking group,with statistically significant differences?P<0.05?.4. Immunohistochemical test results of TIMP-1 in the wound tiss ue of pressure ulcers1)In pressure ulcer healing process of day 1st,day 3rd and day7th,the amount protein expression of TIMP-1 in rats stage?,?in the tissue pressure ulcers were not statistically significant?P>0.05?,a nd day 14th in pressure ulcer healing,the amount protein expression of TIMP-1 in stage?pressure sores organization was significantly be low stage?pressure ulcers,the difference was statistically significant?P<0.05?.2)On day 1,day 3,day 7 and day 14 of the pressure sore heal ing process,there was no significant difference in the protein expressi on level of TIMP-1 in the pressure sore tissues of the smoking group and the non-smoking group?P>0.05?.5.Results of MMP-9/TIMP-1 ratio in wound tissues of pressure ul cers1)In pressure ulcer healing process of day 1,day 3,day 7 and d ay 14,?phase of pressure ulcers in the organization of MMP-9/TIM P-1 ratio is significantly higher than?period pressure sores,differen ces were statistically significant?P<0.05?.2)On day 1,day 3,day 7 and day 14 of the pressure sore heali ng process,the MMP-9/TIMP-1 ratio in the pressure sore tissues of th e smoking group was significantly higher than that of the non-smokin g group,with statistically significant differences?P<0.05?.Conclusion:1.It was clearly proved that passive smoking intervention seriously affected the healing process of pressure sores.2.The imbalance of MMP-9/TIMP-1 ratio is one of the important mechanisms affecting pressure sore healing in smoking rats.3.MMP-9/TIMP-l is expected to be a new target for the treatment of pressure ulcers.In the next research,the clinical application of TIMP-1 will re-balance MMP-9/TIMP-1 to promote the healing of early press ure ulcers.4.The expression level of MMP-9 protein and the ratio of MMP-9/TIMP-1 were related to the degree and stage of pressure sore injury,an d the metabolic balance of matrix metalloproteinase and its inhibitors pl ayed an important role in the stage of pressure sore injury.5.
