Objective:To compare the efficacy and safety of Ultrasound-guided bipolar transurethral plasmakinetic enuclwation of the prostat and transurethral plasma prostatectomy for BPH.The ratio of excised tissue, operative time and intraoperative blood loss were compared. The PVR,Qmax,IPSS and other indicators of the patients were followed up in a short period of time after the operation. Methods:From September,2018-September,2019,82 patients with prostatic hyperplasia were enrolled. The average age was 72.5±11.5 years(65-80 years). The course of disease ranged from 4 to-11 years, with an average of(7.0±1.3)years.A total of 82 patients were divided into two groups using a random number table. The resection group underwent transurethral plasma prostatectomy, and the ultrasound enucleation group underwent Ultrasound-guided bipolar transurethral plasmakinetic enuclwation of the prostat, with 41 patients in each group. There was no statistically significant difference between the two groups(P > 0.05),indicating comparability. Two groups of patients were treated with surgery. The indexes of intraoperative prostate tissue volume, operative time, intraoperative blood loss,Postoperative urinary incontinence, preoperative and postoperative international prostate symptom score, maximum urine flow rate, residual urine volume and so on were compared and analyzed. Results:The two groups of patients were treated with ultrasound-guided plasma enucleation of prostate and transurethral plasma prostatectomy respectively, and the operation was successfully completed. The results showed that the amount of tissue obtained in the ultrasonic enucleation group was more than that in the resection group, and the amount of blood lost in the ultrasonic enucleation group was less than that in the resection group. The international score of prostate symptoms and residual urine volume before and after surgery had clinical advantages, and the differences in various indicators were statistically significant(P < 0.05).There was no significant difference between the two groups in operation time,Postoperative urinary incontinence, maximum urine flow rate and other indicators(P > 0.05).Conclusion :Compared with transurethral plasmoprostatectomy,ultrasound-guided plasma enucleation of the prostate for the treatment of prostatic hyperplasia has a higher proportion of removed tissues,a more accurate postoperative efficacy, and noincreased operative time or surgical risk, which can be further promoted in clinical practice. |
