Objective: 1?A bibliometric analysis was conducted on the literature on the correlation between vitamin D and epilepsy to analyze its research overview and development trend,so as to provide reference for the research on the role of vitamin D in the pathogenesis and treatment of epilepsy.2?To explore the correlation between gene polymorphism of vitamin D Receptor(VDR)and idiopathic epilepsy.3?To explore the correlation between vitamin D level and idiopathic epilepsy and the therapeutic effect of different doses of vitamin D on idiopathic epilepsy.Methods:1?With the database of “big medical medical search”as the retrieval object,all literatures with the theme of “Vitamin D”and“Epilepsy”were searched,and the results were analyzed quantitatively by means of bibliometric analysis,including literature quantity,time distribution,database source,journal species,country,researcher and keyword distribution,etc.2?Through case-control study,91 patients with idiopathic epilepsy were selected as the case group and 105 healthy peoples were selected as the control group.DNA was extracted from peripheral venous blood,and the genotype frequency and allele frequency of Bsm I?Fok I and Cdx2 were detected by polymerase chain reaction(PCR)and Direct sequencing.3?Another 52 patients with idiopathic epilepsy were collected as the case group,and 58 healthy patients in the same period were taken as the control group,and the serum25-hydroxyvitamin D3 content of the two groups was analyzed and compared.4?The 52 patients were randomly divided into 4 groups,which were given vitamin D at 800U/d?400U/d?200U/d and blank group.The clinical efficacy of the 4 groups was analyzed and compared.Results:1?Bibliometric analysis:a total of 385 literatures with the theme of“Vitamin D” and “Epilepsy” were retrieved,and the top three databases were 179 in Pub Med,170 in Web of Science,and 166 in Ebsco MEDLINE Complete.The most prolific journals were Dev Med Child Neuro10(2.6%)and Epilepsy Behavio10(2.6%).The United States ranked first in the number of published articles,with 32(8.31%).The researchers who published the most articles were Alison M Pack,a professor at Columbia University in theUnited States,with 6(1.56%)and Fong C Y,a professor at children's hospital in Brisbane,Australia,with 5(1.30%).“Epilepsy” and “Vitamin D” are the most common in the top ten keywords,followed by “antiepileptic drugs”,“bone”,“safety” and “tolerance”.2?Study on the correlation between vitamin D receptor gene polymorphism and idiopathic epilepsy:(1)The frequency distribution of Bsm I(rs1544410)and Fok I(rs2228570)genotypes in the case group and the control group were consistent with the Hardy-Weinberg equilibrium(P>0.05),indicating that the samples were from a genetically balanced population and were representative of the population.A/G mutation occurred at Cdx2(rs11568820)and the genotype frequency of the polymorphism site was significantly different(P<0.05),and the distribution was not in the hardy-weinberg genetic balance.(2)There was no significant difference in sex composition between case group and control group(?2=0.7952,P=0.373)(3)The t test showed that there was no significant difference in age composition between the case group and the control group(P>0.05),indicating that the two groups matched in age.(4)There was no significant difference in rs1544410 and rs2228570 genotype frequency and allele frequency between case group and control group(P >0.05).(5)In genotype sequencing,there was a significant difference in genotype frequency(?2=6.67,P=0.036)between the case group with gene locus rs11568820 and the control group,but no significant difference in allele frequency.(6)In the association analysis of rs11568820 genotype and idiopathic epilepsy,the AG genotype of rs11568820 was the risk factor of idiopathic epilepsy(OR=2.222,95%CI:1.190~4.150,P=0.027).3?The correlation between the level of Vitamin D in vivo and idiopathic epilepsy :Vitamin D levels were significantly lower in the case group than in the control group.4?Effect of different doses of Vitamin D on idiopathic epilepsy:(1)There was a statistical difference in the total effective rate between the high-medium dose group and the blank group(P<0.05).(2)There was no significant difference in the total effective rate between the low-dose group and the blank group(P>0.05).(3)There was no significant difference in the total effective rate between the high andmedium dose groups(P>0.05).(4)There was a statistical difference in the total effective rate between the medium and low dose groups(P<0.05).Conclusion: 1?The research on the correlation between vitamin D and epilepsy has been paid more and more attention.There are few research achievements in this field in China,so it is necessary to strengthen the research in this field.2?The polymorphism of Vitamin D receptor gene Cdx2(rs11568820)may be related to the susceptibility of idiopathic epilepsy.3?Vitamin D levels are reduced in patients with idiopathic epilepsy.4?Daily vitamin D of 400 U may improve the symptoms of patients with idiopathic epilepsy.
