Objective: By reporting a case of klebsiella pneumoniae liver abscess(KPLA),combining its clinical characteristics,imaging manifestations,pathogen examination and clinical diagnosis and treatment process,the clinical diagnosis and treatment ideas of KPLA were summarized,so as to improve clinicians' understanding and diagnosis level of KPLA.At present,klebsiella pneumoniae(KP)has surpassed Escherichia coli to become the most common pathogen of bacterial liver abscess in China,and the hypervirulent Klebsiella pneumoniae(hv KP)liver abscess is often merge extrahepatic disseminated infection,which progress quickly,and the prognosis is poor.In recent years,the detection rate of multi-drug resistance and high toxicity KP gradually increases,which brings great challenges to the diagnosis and treatment of clinicians.This article will focus on the epidemiology,pathogenesis,high virulence and high drug resistance,clinical characteristics,etiological detection and diagnosis and treatment of hv KP liver abscess.Methods: The clinical data of a case of KPLA were retrospectively analyzed,and the progress of diagnosis and treatment of KP,especially hv KP liver abscess was summarized in combination with domestic and foreign literatures.Results: The main clinical manifestations of this patient were fever,and chills without obvious causes.According to the history,signs,imaging examination and laboratory examination,the diagnosis was as follows: 1.Liver abscess(ESBL positive klebsiella pneumoniae infection);2.Cholecystitis cholecystolithiasis;3.Pulmonary infection;4.Coronary atherosclerotic heart disease;5.Arrhythmia,atrial fibrillation;6.Hypertension level 3 high risk.Conclusions: The incidence of KPLA is increasing year by year,and multiple drug resistant,hypervirulent KP strain detection rate is growing up.The clinical symptoms of KPLA patients are mostly asymptomatic.However,patients with hv KP liver abscess often have extrahepatic disseminated infection,with severe condition,rapid progress and poor prognosis.Therefore,clinicians need to be proficient in the clinical and imaging characteristics of KPLA,especially hv KP liver abscess,so that early identification,early diagnosis,and standardized treatment are required.At the same time,physicians should be alert to symptoms that may occur during treatment.If necessary,please combine treatment with related departments to improve patient prognosis.
