| Objective:(1)To study the changes of TCM syndrome type,echocardiographic index and nt-probnp level in hypertensive patients with diastolic dysfunction,so as to provide objective reference basis for syndrome differentiation and treatment(2)To study the efficacy and mechanism of Fengxuan III prescription in treating patients with Yin deficiency and Yang hyperactivity syndrome,and further verify the correctness of the correlation study.Methods: From July 2018 to July 2019,a total of 78 patients with hypertension complicated with diastolic dysfunction who met the requirements and were admitted to the outpatient and inpatient departments of the second department of cardiology of sichuan hospital of traditional Chinese medicine were included.The syndrome types in traditional Chinese medicine are four syndromes: irascitis,phlegm-dampness,Yin deficiency,Yang hyperactivity and Yin and Yang deficiency.The cardiac ultrasound index and nt-probnp level of each syndrome type were statistically analyzed to explore the relationship between the former and the TCM syndrome type of hypertensive diastolic dysfunction.To observe the changes of echocardiographic indexes in patients with Yin deficiency and Yang hyperactivity syndrome before and after treatment with fengxuan prescription III,and to analyze them by statistical methods.Results:(1)study of correlation:(1)There was no significant difference in the distribution of patients' gender and age,so there was comparability among the syndromes;(2)Cardiac color doppler ultrasound structure index results showed that the morphological indexes of left ventricle of the four syndrome types,including LVEDd,IVS and PWT,were all normal without significant difference(P > 0.05);(3)The results of ventricular function indexes showed that LVEF of all syndrome types was normal,and the differences between Yin deficiency,Yang hyperactivity and Yin and Yang deficiency and other syndrome types were significant(P<0.05).The diastolic function indicators E,E ',E/ E ',A,E/A and TRPV showed significant differences in E,E ',E/ E ',A and E/A(P<0.05).There was no available diversity in TRPV(P>0.05);(4)The criterion of ntpro BNP in each syndrome style has a available diversity(P<0.05),successively for Yin and Yang two asthenia,phlegm-dampness accumulate,Yin asthenia Yang hyperactivity,the sequence of irregularity on the fire gradually decreases.(2)Clinical efficacy study: after treating patients with Yin deficiency and Yang hyperactivity syndrome with fengdizzy prescription III,there was no significant change in the ventricular structure index of patients with color doppler ultrasound(P > 0.05),and the systolic,diastolic function,ntprobnp level,blood pressure and syndrome score were significantly improved(P < 0.05).Conclusion:(1)In patients with hypertension complicated with diastolic dysfunction,there was a good correlation between TCM syndromes and the changes of cardiac color doppler ultrasound indexes and plasma nt-probnp level.In clinical,the type of syndrome can be determined by combining the changes of symptoms,and objective indicators such as cardiac structure,systolic and diastolic function,and nt-probnp level.(2)After treatment with wind anti-dazzle III,party of Yin hand syndrome in patients with the ultrasonic cardiogram index and plasma NT-pro BNP significantly,compared with before treatment changes shows that the wind anti-dazzle III,can significantly improve patients with left ventricular diastolic dysfunction,heart colour to exceed and supporting the plasma NT-pro BNP index for patients with hypertension combined diastolic dysfunction syndrome identifying and judging has certain specificity,good correlation with clinical syndrome and further confirmed that the merger of high blood pressure,diastolic dysfunction in patients with TCM syndrome and heart colour to exceed the indexes and the level of plasma NT-pro BNP close relationship.At the same time,it also explains the accuracy of our clinical syndrome differentiation. |
