| Objectives The aim of this study is to analyze the imaging and clinical data of patients with acute IPI,to explore the etiology and influencing factors of progressive aggravation in IPI patients,and to provide evidence for the optimization and prevention of treatment strategies.Methods Patients admitted to the ward of neurology department of Tangshan Workers Hospital affiliated to North China University of Science and Technology from January 2017 to December 2019 and confirmed to be IPI by the head DWI were collected.The subjects that met the inclusion criteria were PIPI patients whose NIHSS score increased by more than 2 points in one week compared with the time of admission,and the NPIPI patients whose NIHSS score did not change or decrease.The clinical data and laboratory indicators of the patients were collected,and the imaging characteristics of the lesion sites and diseased vessels were analyzed by DWI and HRMRI,and the etiological classification was conducted.Related factors of PIPI were screened out by univariate analysis,and independent risk factors of PIPI were determined by multivariate Logistic regression analysis.Results A total of 44 IPI patients were collected,including 15 in the PIPI group and 29 in the non-PIPI group.1 There were no statistically significant differences in gender and age between the two groups(P>0.05)in general data,indicating comparability.2 Single factor analysis results: BMI,NIHSS score on admission,m Rs score upon discharge,ESR,DWI involving more than 2 layers can,involving the pons ventral surface,the largest long diameter of the DWI terrier,VP,the narrowest level diameter,PA,PB,vascular stenosis rate,plaque on the right side,etiology classification comparison between the two groups,the difference had statistical significance(P<0.05);History of stroke,coronary heart disease,diabetes,smoking history,stroke history,drinking history,history of hypertension,systolic pressure,diastolic pressure,hospitalization days,blood glucose on admission,WBC,RBC,hemoglobin,platelet count,erythrocyte deposited,total cholesterol,triglyceride,LDL-C,HDL-C,hcy,plasma viscosity,fibrinogen,D-dimer,hypersensitive c-reactive protein,infarcts located in the center of the right,next to the pons infarction,infarcts maximum transverse diameter,DWI infarcts in distribution,between the two groups can compare,there were no statistically significant difference(P>0.05).3 Multivariate Logistic regression analysis: the etiology was classified as LCAD and NIHSS score at admission was an independent risk factor.Conclusions 1 BMI,NIHSS score on admission,m Rs score upon discharge,ESR,DWI involving more than 2 layers can,involving the pons ventral surface,the largest long diameter of the DWI terrier,VP,the narrowest level diameter,PA,PB,vascular stenosis rate,plaque on the right side,etiology classification are the influencing factors that may contribute to the aggravation of IPI.2 Etiological classification was the type of LCAD and NIHSS score at admission was an independent risk factor for PIPI.Figure5;Table11;Reference 157... |
