Objectives To explore the relationship between the epidermal growth factor receptor(EGFR)mutation status of peripheral blood circulation tumor DNA(ct DNA)and clinical features of peripheral lung cancer and its clinical significance,to provide targeted therapy for peripheral lung cancer Diagnosis and treatment ideas.Methods A total of 55 patients with peripheral lung cancer admitted to the Affiliated Hospital of North China University of Technology from March 2018 to September 2019 were included in this study.The peripheral veins of 55 patients with peripheral lung cancer were applied with droplet dijital PCR(dd PCR)Blood is tested for EGFR mutation status of ct DNA,and its tumor tissue is also tested for EGFR mutation,to collect basic clinical information of patients,to improve the D-dimer,erythrocyte sedimentation rate,tumor markers,Chest high-resolution CT and other auxiliary examinations.SPSS17.0 statistical software(version 17.0,IBM)software was used for data compilation to establish a database and perform statistics and analysis.For quantitative data conforming to normal distribution,(sx ±)was used for statistics Description,independent group t test was used to compare the mean between groups.The percentage(%)of the qualitative data is calculated,and the comparison of the composition ratio between the groups is by the ?2 test or Fisher test.Multivariate analysis uses Logistic regression model.The test level was a=0.05,P <0.05 was considered statistically significant.Results 1 There was no significant difference in the mutation rate and type of EGFR gene between peripheral blood ct DNA and tumor tissue(P>0.05).2 By detecting the mutation of EGFR gene in peripheral blood ct DNA and tumor tissue,it was found that the detection results of the two samples were highly consistent(kappa=0.925).3 The differences of sex,smoking history,histopathology and pathology between the EGFR gene mutation group and the wild type group were statistically significant(P<0.05);the differences of age,family history of lung cancer and TNM stage were not statistically significant(P>0.05).4 There was no significant difference in CEA,CA199,NSE,D-dimer and ESR between the two groups(P>0.05).5 There were significant differences between the two groups(P<0.05),but there were no significant differences in tumor size,tumor location,burr sign,satellite focus,vascular bundle sign,cavity,lobulation sign and bronchial truncation sign between the two groups(P>0.05).6 The results of multivariate logistic regression analysis showed that: female,non-smoking,adenocarcinoma,pleural indentation were closely related to EGFR gene mutation(P<0.05),which was the risk factor of EGFR gene mutation in peripheral blood ct DNA.7 After treatment with EGFRTKIs,the ORR and DCR of EGFR mutant patients were significantly higher than those of EGFR wild type patients(P<0.05).Conclusions 1 When the tumor tissue can not be obtained,the peripheral blood ct DNA can replace the tumor tissue as the specimen of EGFR gene detection to some extent.2 The risk of EGFR gene mutation in peripheral blood ct DNA of female,non-smoking,adenocarcinoma and peripheral lung cancer patients with pleural indentation is increased,which is the dominant population to select peripheral blood ct DNA for EGFR gene detection.3 The EGFR gene status of peripheral blood ct DNA is a predictor of the therapeutic effect of lung cancer targeting therapy.Figure0;Table12;Reference 127...
