Objective:1.Analyze the accuracy and feasibility of carotid artery stenosis assessment with magnetic resonance black blood technology;2.To investigate the accuracy and feasibility of evaluating the stability of carotid plaque before surgery by magnetic resonance black blood technology,and provide theoretical basis for medical and surgical treatment.Methods:1.Select the patients diagnosed with carotid stenosis in our hospital from May 2018 to July 2019,observe the results of magnetic resonance black blood imaging examination and related CT angiography images,and compare and evaluate the degree of carotid stenosis Sex.2.Collect the MRI black blood test results of patients undergoing carotid endarterectomy during this period,observe the MRI black blood test results of carotid plaque,determine the plaque stability,and analyze the consistency of the results with the pathological results.3.Statistical analysis was performed using the SPASS17.0 statistical software package,and the authenticity and reliability(Kappa)of the two monitoring methods of magnetic resonance black blood technology and CT angiography were statistically analyzed;the measurement data were using chi-square test and exact probability method Calculate the P value(n <40),and the test level is p = 0.05.Results:1.MRI black blood results: 13 branches with mild stenosis,23 branches with moderate stenosis,18 branches with severe stenosis,and 2 completely occluded;CTA results: 13 branches with mild stenosis,23 with moderate stenosis,18 branches with severe stenosis,and complete occlusion Two;the magnetic resonance black blood test results and the CTA test results are more consistent in assessing carotid stenosis,Kappa value> 0.75(Kappa = 0.785),the difference is not statistically significant.2.Pathological results: 21 plaques showed a fibrous cap structure,18 had a lipid core,13 plaques had bleeding,and 15 plaques had calcification;magnetic resonance black blood technology results: 19 fiber cap signals and 15 lipid cores There were 12 branches with plaque bleeding and 13 calcifications.There was no statistically significant difference in the detection of plaque fiber cap between magnetic resonance black blood and pathological results,P> 0.05(P = 0.688);both were within the detected plaque.There was no significant statistical difference in the lipid core,P> 0.05(P = 0.375);there was no significant statistical difference between the two in detecting plaque bleeding,P> 0.05(P = 0.625);the two were in detecting plaque calcification There was no significant statistical difference,P> 0.05(P = 0.727).Conclusion:1.Magnetic resonance black blood technology can achieve the effect of CTA in judging carotid stenosis,especially its non-invasive,non-radiation,no need to inject contrast agent,etc.,it is worthy of vigorous promotion and application in clinical.2.MRI black blood technology has a good consistency with pathology in assessing the stability of carotid atherosclerotic plaques,and it has very important guiding significance for related treatments such as surgical CEA and standard medical treatment.
