Objective:Interstitial lung disease(ILD)represents a group of lung diseases,often with diffuse,multifocal lung changes.At present,ILD is recognized as a disease that is difficult to diagnose and treat at home and abroad,and the diseased population is gradually becoming younger,which has long puzzled medical workers.This study compared the serum matrix metalloproteinase-7(MMP-7)and diffusing capacity of the lung for carbon monoxide(DLCO)as a percentage of estimated value(%)in different types of ILD patients and healthy controls.The purpose of this study is to observe the differences in the levels of different types of ILD patients and healthy people,to explore its clinical significance,and to provide guidance for the early prediction and diagnosis of ILD,disease assessment,and prognosis.Methods:This project used a prospective research method to collect ILD patients who were hospitalized in the Department of Respiratory Medicine of Chengde Central Hospital for the first time from November 2017 to October 2019,and then were clearly classified in Chengde Central Hospital or the upper level hospital,a total of 52 example.25 patients with known causes of ILD were in group A;18 patients with idiopathic interstitial pneumonia(IIP)were in group B;9 patients with granulomatous ILD were in group C.Twenty healthy retirees who were examined at the Medical Center of Chengde Central Hospital were selected as the control group,which was called group D.Collecting basic information of 72 subjects(Includes information such as gender,age,comorbidities and complications),determination of serum MMP-7 levels and DLCO%.Analysis of serum MMP-7 and DLCO% in each group of study subjects.SPSS21.0 statistical software is used for data processing and statistics.The measurement data conform to the normal distribution and are expressed by mean(x ±s)standard deviation.Data analysis using single factor analysis of variance.Data conversion that does not meet the normal distribution,if it still does not meet the normal distribution,it is expressed by the median and interquartile range,that is,M(q),using a non-parametric test.Pearson correlation analysis is used for data that meets the normal distribution,and Spearman correlation analysis is used for data that does not meet the normal distribution.And the counting data is tested by Chi square test.The set test level was 0.05,and P < 0.05 was significant difference.Results:1.general informationThere were 78 researchers in this experiment,25 patients in group A(8 males and 17 females),average age(62.72 ± 6.64)years;18 patients in group B(8 males,10 females),average age(63.28 ± 6.20)years;9 patients in group C(3 males and 6 females),average age(60.11±3.48)years;20 healthy people in group D,(10 males and 10 females),average age(60.90±4.12)years.There was no significant difference in age and sex between four groups(P>0.05).2.Serum MMP-7 levels in four groupsThe serum MMP-7 level in group A was 0.282±0.021 ng /ml,the serum MMP-7 level in group B was 0.3101±0.021 ng /ml,the serum MMP-7 level in group C was 0.277±0.011 ng /ml,and in group D The control group serum MMP-7level was 0.230±0.025ng/ml.There was n o significant difference in serum MMP-7 levels between group A and group C,and there were statistically significant differences in the level s of MMP-7 in the remaining serum(P<0.05).3.DLCO% in four groupsThe DLCO% in group A was 45.90 ± 4.20%,the DLCO% in group B was 46.61 ± 4.32%,the DLCO% in group C was 47.73±2.76%,and the DLCO% in group D was 91.38 ± 3.74%.The comparison of DLCO% between group A and group C showed no statistically signifi cant difference(P>0.05).The comparison of DLCO% among the remai ning groups showed a difference and was statistically significant(P<0.05).4.Correlation between serum MMP-7 and DLCO%Correlation analysis showed that DLCO% was negatively correlated with serum MMP-7(P<0.05).Conclusion:1.The serum level of MMP-7 in patients with ILD is higher than that in healthy people.It is inferred that MMP-7 can provide a certain degree of promptness for the diagnosis of ILD.2.The patients with idiopathic interstitial pneumonia had the worst DLCO% and the most significant increase in serum MMP-7 levels,suggesting that MMP-7 levels may be higher than those during inflammation during the fibrotic matrix remodeling phase.The stage of development of ILD patients can be judged by dynamically observing MMP-7 levels.3.DLCO examination is a sensitive and effective method for early assessment of lung function impairment in ILD patients.The results of this experiment show that DLCO% in ILD patients is lower than that in healthy controls.Serum MMP-7 and DLCO% are significantly negatively correlated,suggesting that serum MMP-7 may be serious as ILD Reference indicator of degree.Especially for patients who cannot cooperate with the completion of lung function test.
