ObjectiveTo observe the change of Transglutaminase 2 Tissue(TG2)levels in serum and urine of children with Henoch-Schonlein purpura in the acute stage,and to explore its role in children with purpura nephritis.MethodsSelect 91 cases of Henoch-Schonlein purpura(HSP)children as the research object who were treated in Children's Medical Center,Affiliated Hospital of Qingdao University from February 2019 to December 2019.Depend on whether who were combine with renal injury,91 cases were divided into HSP without renal injury group(NHSPN,52 cases)and HSP renal injury group(HSPN,39 cases).Depend on whether who were combined with albuminuria,then HSPN group was divided into albuminuria group and without albuminuria group.At the same time,select 30 cases of children in the same age who came to our hospital for health examination in child healthcare department as normal group.Enzyme-linked immunosorbent assay(ELISA)was used to detect the serum and urine levels of TG2 in children in each group?SPSS22.0 statistical software was used for statistical analysis,and variance analysis was used to compare the differences of TG2 levels in each group.Meanwhile,the 24-hour urinary protein quantitative level in children with HSPN was detected.Correlation analysis of serum TG2 and urine TG2 levels,24-hour urinary protein quantitative levels and urine TG2 levels was conducted.ROC curve method was used to estimate the sensitivity and specificity of urinary TG2 value in the diagnosis of HSPN.Results1?Changes of the serum TG2 level in children in each group: There was no significant difference in serum TG2 levels between HSPN group,NHSPN group and normal control group(P>0.05).2?Changes of the urine TG2 level in children in each group:The level of TG2 in the urine of HSPN group was significantly lower than that of the normal control group and NHSPN group(P<0.01;P<0.01),and the difference was statistically significant(P<0.05).3?There was no correlation between serum TG2 level and urine TG2 level in HSPN group(P>0.05)4?In the HSPN group,the urinary TG2 level was negatively correlated with the24-hour urinary protein level(P<0.05).Difference was statistically significant in the urine TG2 level between the proteinuria group and the non-proteinuria group(P<0.05).5?When the urinary TG2 value of children with allergic purpura was ?0.316(ng/ml),the sensitivity of HSPN were 0.795,and the specificity of HSPN were 0.922.Conclusions1?The urinary level of TG2 in the HSP nephropathy group(HSPN)was significantly lower than that in the HSP non-nephropathy group(NHSPN)and the normal control group,and that was negatively correlated with 24-hour urinary protein quantification.2?The TG2 index in urine may be the biological marker of purpura nephritis,which was of certain guiding significance for the early evaluation of children with allergic purpura with renal injury.
