Objective This article mainly used the magnetic resonance IDEAL-IQ sequence to quantitatively measure the adult lumbar vertebral bone marrow and paraspinal muscle fat content, and analyzes its correlation with age and gender, and understand its changing trend with age and gender. The aim is to provide new clinical solutions for the diagnosis, treatment and evaluation of diseases affecting the vertebral bone marrow and paraspinal muscle fat content. Methods The study subjects selected 2018.6-2020.1,120 adult volunteers who were examined at Qingdao Municipal Hospital.Scanning method, using GE 3.0T Discovery MR 750 magnetic resonance imaging system, spinal phased array coil,120 volunteers who meet the inclusion and exclusion criteria are taken in the supine position, the head is advanced, and the sagittal position of the lumbar vertebrae and the paraspinal muscles between the 4 and 5 vertebrae are performed by IDEAL-IQ sequence. In sagittal scan, the lumbar 3 vertebra was used as the scan center, the scan range was from the upper margin of the vertebral body of thoracic 11,and to the lower margin of the sacral 2 level. When scanning the transverse axis between lumbar 4 and 5 vertebral bodies, the positioning line should be parallel to the lower edge of lumbar 4 vertebral body and the upper edge of lumbar 5 vertebral body, and the scanning range should be from the lower part of lumbar4 vertebral body to the upper part of lumbar 5 vertebral body. For the region of interest drawing, use the GE ADW 4.6 workstation Functool software to process the sagittal and transverse axis images of the lumbar spine obtained from the IDEAL-IQ sequence scan.In the mid-sagittal position of the sagittal fat fraction map, that is,the largest level of the vertebral body, delineate the region of interest of the lumbar 1 to 5 vertebrae. The interested area of paraspinal muscle was delineated on the axial water map with no obvious fat infiltration and clear muscle contour, and was replicated on the fat fraction map at the same level. The average fat fraction of paraspinal muscle on the left and right sides was taken as the final measurement value. All measurement data(vertebral fat fraction and paraspinal muscle fat fraction)are expressed as x±s.Univariate analysis of variance(ANOVA)was performed for the difference in fat fraction of the same vertebral body in different age groups, analysis of differences in vertebral and paraspinal fat fractions between different genders of the same age group using two independent sample T tests, pearson correlation analysis was used to analyze the age-related relationshipbetween vertebral and paraspinal muscle fat fractions in male and female volunteers. All statistical analyses were performed at P <0.05,the difference was statistically significant. Results A one-way analysis of variance of the same vertebral body fat fraction in different age groups of male and female found that there was a statistically significant difference in the same vertebral body fat fraction between different age groups,P <0.05.Pearson correlation test was performed on the average fat fraction of L1 to L5 vertebral body of all males and females, the results showed that the fat content of L1 to L5 vertebral body was positively correlated with age, among them, the r values of the vertebral fat fraction from L1 to L5 in female were r1=0.79,r2=0.79,r3=0.78,r4=0.82,r5=0.81,all P values were <0.05,while the r values of the vertebral fat fraction from L1 to L5 in male were r1=0.63,r2=0.61,r3=0.70,r4=0.71,r5=0.72,all P values were <0.05.For the comparison of vertebral body fat fraction between different genders in the same age group, the vertebral body fat fraction of males in the 21-40 age group was higher than that of females, the vertebral body fat fraction of males in the 41-60 age group was slightly higher than that of females, and the vertebral body fat fraction of females in the61-80 age group was slightly higher than that of males. Two independent sample t tests showed that P= 0.007 in the 21-40 age group, with statistically significant differences,in the 41-60 age group and the 61-80 age group, both P values were >0.05,with no statistically significant differences.Pearson correlation test was conducted for the average fat fraction of all vertebral bodies in male and female with age, which was positively correlated with age,r=0.81 for female and r=0.69 for male, and all P values were <0.05.Postmenopausal women's vertebral fat score is significantly higher than that of premenopausal women's vertebral fat score.Pearson correlation tests were performed on male and female L4 / L5 levels of paraspinal muscle fat fractions with age,and both were positively correlated with age, female r = 0.79,male r = 0.74,and P values were all <0.05.For the comparison of paraspinal muscle and vertebral fat fraction between different genders in the same age group, the overall paraspinal muscle fat fraction of females was higher than that of males, two independent sample t-tests were performed at P = 0.253 in the 21-40 age group, and the difference was not statistically significant, the P-values of the 41-60 age group and the 61-80 age group were both <0.05,and the difference was statistically significant. Conclusions 1.3.0T MR IDEAL-IQ sequence is a new technology that can quantitatively measure the fat content of vertebral body and paraspinal muscle.2.The bone marrow fat content of the same vertebral body was different in different age groups,in the same age group, the lower vertebral body(L4,L5)fat content was higher than the upper vertebral body(L1,L2).3.The lumbar vertebral bone marrow and paraspinal muscle fat content is positively correlated with age,the older the lumbar vertebral body and the paraspinal muscle fat content value, the higher the correlation between female and age.4.In thesame age group, there are differences in vertebral and paraspinal fat fractions between different genders.5.The bone marrow fat content of lumbar vertebrae in postmenopausal women was significantly higher than that in premenopausal women.
