Background and purpose: Hp is a globally infectious chronic pathogenic bacteria that is mainly transmitted orally and colonized in the gastric mucosa of more than50% of the population.Clinical studies have confirmed that Hp is associated with peptic ulcer,chronic gastritis,and gastric mucosa Lymphoma and gastric cancer are closely related and are one of the important factors leading to gastric lesions.They are classified by the WHO as a class I carcinogen and a risk factor for gastric cancer.Digestive system diseases are common diseases that affect human health.With the deepening of research on digestive system diseases,Hp has become more prominent in the prevention and treatment of digestive system diseases.Therefore,the detection of Hp is of great significance for early detection and treatment.Therefore,the establishment of a rapid,sensitive and specific diagnostic method for H.pylori infection can provide a basis for clinical diagnosis,treatment,efficacy observation and prognosis determination.This study investigated the correlation between histopathological methods of H.pylori infection and endoscopic findings,To explore the clinical diagnosis value of NBI-ME mode endoscopy for gastric mucosal microlesions.Methods: A random selection of 280 patients with chronic gastritis diagnosed by gastroscopy from June 2016 to May 2019 at Peking University Medical College of Traditional Chinese Medicine was performed.Local anesthesia and lubrication were performed on the patients.A comprehensive exploration was performed by a competent gastroscopy physician in the white light mode first,and then the magnification and NBI mode observations were determined in turn.According to the microscope,the small stomach depression and the collecting vein were typed.The histopathological examination method was to cut the mucosal tissues of the patient’s gastric body(about 8 cm away from the cardia),the small middle side of the stomach(about 3 cm from the pylorus),and the small curvature side of the gastric antrum.The Hp was determined by HE staining and Warthin-Starry silver staining,high magnification.Hp colonization of gastric pits in 10 fields of each sample was observed under a microscope.To observe the relationship between microstructure and pathological types of gastric mucosa and Hp infection under narrow-bandimaging magnification endoscopy(NBI-ME),and to explore the correlation between histopathological methods of Helicobacter pylori infection and endoscopic performance Results:(1)Type A of small pits was regular dots;type B was short rods;type C was sparse pits;type D was plaques;pits E were villous;Collection of venous morphology R-type regular type,I-type irregular type,D-type disappearing type;A type will only appear in the gastric body and fundus,and generally no obviousinflammation and other lesions;B type is mainly distributed in the gastric antrum,It usually does not show obvious inflammation and other lesions;Type C mainly appears in the mucosa with inflammation,edema and other lesions;Type D mainly appears in the area of obvious inflammation,edema,and rot;Type E only appears in the intestinal area.In these five forms,they can cross each other.(2)Of the 280 cases,129 cases were diagnosed as Hp-related gastritis by pathological examination.The highest H.pylori infection rate in the microstructure of the lower gastric depression and lateral collecting veins of NBI-ME was C+D,with a positive rate of 86.84%,which was statistically significant compared with other structures(P<0.05).H.pylori infection rate in pathological types The highest was the intestinal metaplasia type,with a positive rate of 53.45%,which was not statistically significant compared with other types(P> 0.05);the coincidence rate of NBI-ME gastroscopy diagnosis and pathological diagnosis was 98.57%.Histopathological examination showed that 4 cases were diagnosed as false positive by gastroscopy,and the microstructure of gastric pits and lateral collecting veins were both C+R type.Conclusions:(1)The morphology of gastrointestinal pits and collecting veins is closely related to histopathology.NBI-ME observed the microstructure of the gastrointestinal pits and collecting veins.The positive rate of H + infection was the highest in C+D infection.(2)After Helicobacter pylori infection,the morphology of vein collection changed.(3)In clinical practice,attention should be paid to the combination of new gastroscopy techniques and pathological examinations to strengthen morphological understanding under gastroscopy and standardize biopsy of typical sites to improve the diagnostic coincidence rate of the two.
