Purpose:Parathyroid hormone?PTH?is a hormone that regulates calcium and phosphorus metabolism and bone conversion.Previous studies have found that patients with newly diagnosed multiple myeloma?MM?often have increased PTH content[1].A large number of studies have shown that the occurrence and development of some cancers?ovarian cancer,lung cancer,bone metastasis,kidney cancer,etc.?are closely related to PTH level[2-5].The purpose of this study was to investigate the significance of serum PTH in patients with multiple myeloma and its relationship with other risk factors for multiple myeloma.Methods:This study selected 107 newly diagnosed multiple myeloma patients from the Hematology department of Shengjing Hospital from 2017 to 2019.According to the PTH value at the initial diagnosis,the patients were divided into elevated group and non-elevated group.The different clinical parameters and PFS?progression-free survival?of the 2 groups were compared,and the correlation between PTH level and different clinical parameters and PFS,the relationship between PTH and other clinical parameters,and the influence of PTH elevation on the survival time of MM were calculated.Result:The levels of serum calcium and peripheral blood leukocytes were lower in the group with elevated PTH,and the difference between the groups was statistically significant?P<0.05?.There was no significant difference in general data,monoclonal protein type?table 1.3?between the two groups?table 1.1?,no significant difference in multiple myeloma-related laboratory examination Group?table 1.4?,and no significant difference in treatment effect of IMWG?International Myeloma Working Group??table1.5?.Pearson correlation analysis results suggested that PTH level was negatively correlated with serum calcium?R=-0.270,P=0.005?,white blood cells?R=-0.195,P=0.044?,and positively correlated with urea?R=0.222,P=0.022?,creatinine?R=0.241,P=0.012?,and albumin?R=0.191,P=0.048?.According to COX survival analysis,the risk of progression in the non-elevated PTH group?<88pg/ml?was 0.523 times higher than that in the elevated PTH group??88pg/ml?.The time of PFS in patients with multiple myeloma with elevated PTH was shorter than that in patients with non-elevated PTH?P=0.012?.Conclusion:PTH value at the initial diagnosis was a risk factor for PFS in patients with multiple myeloma.Patients with elevated PTH had shorter PFS.PTH is related to the regulation of blood calcium in patients with multiple myeloma.There is a negative correlation between PTH value and leucocyte level in peripheral blood of patients with multiple myeloma at the time of initial diagnosis,but the effect on leucocyte function and immune system still needs to be further studied.
