A Study On The Current Situation Of The Construction Of Medical Consortia In A Third Class A Hospital In Shenyang

Objective: For a long time,there are many problems in China's medical and health undertakings.In order to allocate medical resources more reasonably,the country has carried out continuous medical reform in the past decade,and the hierarchical diagnosis and treatment system has become the first of the five basic medical and health systems in the new era.In order to promote the formation of the hierarchical diagnosis and treatment system,China is currently building a variety of forms of medical consortia,with three-level public hospitals playing a leading role,and enhancing the service capacity of the grass-roots level,so as to facilitate the masses to seek medical treatment nearby.This paper explores the specific construction mode and achievements of a hospital medical complex.According to the research results,the advantages and disadvantages of the medical consortium are analyzed to provide scientific basis for the construction of the medical consortium.Methods: In this study,descriptive research was used to analyze the change of the basic construction indicators of each member unit by the governance mechanism,organizational management and other basic models of the medical consortium of hospital a.Chi square test was used to compare the changes of human resource composition of each member unit before and after the implementation of the Medical Association;weighted TOPSIS comprehensive evaluation method was used to evaluate the indicators of the Medical Association,so as to judge whether the formation of the medical association can promote the development and construction of the hierarchical diagnosis and treatment model.Results: The medical consortium of hospital a was formally established as early as April 2018.Hospital a and its member units have changed their human resource structure after the establishment of the medical Consortium;some indicators of the subordinate units have been improved in terms of service volume,medical service efficiency and social benefits.There are reasonable rules and regulations for the first diagnosis and the separation of the urgent and the slow treatment in the medical consortia.The number of two-way referrals of hospital B in the year when the medical consortia was established was 4.89 times that of the previous year,and thenumber of two-way referrals of community health service center C was 2.34 times that of the previous year.B hospital received 456.5 times of business guidance,174 person times of personnel training,C community health service center received 242.5and 388 times of business guidance,and the total number of personnel training was118 and 107 respectively.Since the official operation of the remote image platform,8552 cases of remote image diagnosis have been completed as of December 31,2019.The construction of the remote image platform will promote the internal construction of the medical consortium and the formation of the hierarchical diagnosis and treatment system.According to the comprehensive evaluation,the CI value of B hospital and C community health service center increased from 0.599 to 0.919 and0.039 to 0.915 respectively.The establishment of the medical consortium promoted the construction of the hierarchical diagnosis and treatment service system in the region where the medical consortium is located.Conclusion: The close medical association in this study has formed a perfect governance mechanism and management mode,which generally seems to play a positive role in helping lower level hospitals.To a certain extent,the establishment of medical consortia improves the capabilities of primary medical institutions,but significant improvement still needs time to accumulate.In the construction of the hierarchical diagnosis and treatment system,the upper and lower referral,acute and slow treatment have achieved certain results.The effect of the first diagnosis at the grass-roots level is not obvious,but the number of outpatients and inpatients in the lower level medical institutions has been increased through effective up-down linkage and other ways.The construction of remote image platform can promote the internal construction of medical consortia and the formation of hierarchical diagnosis and treatment system.The comprehensive evaluation results show that the CI value increases significantly after the completion of the medical consortium,indicating that the establishment of the medical consortium is conducive to the construction of the hierarchical diagnosis and treatment system.
