Diagnosis Of Thyroid Nodules Using Radiomics Models Applied To Sonographic Images: A Prospective,Diagnostic Study

Objective : This study was performed to evaluate the feasibility of ultrasonic radiomics models in differentiating benign and malignant thyroid nodules and compare it with TIRADS and elastic grading of nodules evaluating by senior doctors to evaluate the diagnostic value of the radiomics models.Materials and Methods: A total of 74 patients with thyroid nodules were collected from the Cancer Hospital of China Medical University who were treated surgically or confirmed by pathology from November 2018 to August 2019,including 32 cases of benign nodules and 42 cases of malignant nodules.Two-dimensional gray-scale images and elastic images of the nodule were collected,while TI-RADS and VTI were evaluated by senior ultrasound doctors.At the same time,two-dimensional images and elastic VTI images were used for radiomics analysis.SPSS 24.0 and medcalc software were used for statistical analysis.Logistic regression was used to establish four models(TI-RADS + VTI model,two-dimensional radiomics model,VTI elastic radiomics model,and two-dimensional + VTI elastic radiomics model).The sensitivity,specificity,accuracy and AUC of the four models were calculated.Delong test was used to compare AUC of different models.Results: The sensitivity,specificity and AUC of the four models were 76.19%,84.37%,0.86;90.48%,78.12%,0.89;92.86%,53.13%,0.73;73.81%,100.00%,0.93,respectively.Delong test was used to compare the AUC of four models.The results showed that the diagnostic efficiency of TI-RADS and VTI radiomics model was similar to that of two-dimensional group model and two-dimensional + VTI elastic radiomics model,and the difference of AUC was not statistically significant.Conclusion: The ultrasonic radiomics model can be used to distinguish benign and malignant thyroid nodules and has a high diagnostic value.The diagnostic efficiency is equivalent to that of senior doctors,and it is helpful and valuable for doctors to make clinical decisions.
