Abstract/Summary:
Objective:To observe and analyze the thyroid function in patients with type 2 diabetic kidney disease,and explore the effect of thyroid function on the incidence of diabetic kidney disease.Methods:A total of 150 patients with type 2 diabetes admitted to the 2nd Department of Endocrinology,Shengjing Hospital from January 2015 to September 2019 were selected.According to the diagnostic criteria of diabetic kidney disease,it was divided into DKD group(patients with diabetic kidney disease)and NDKD group(patients without diabetic kidney disease).According to the urine albumin-to-creatinine ratio value,it was divided into A0 group(urine albumin-to-creatinine ratio<30 mg/g),A1 group(30 mg/g≤urine albumin-to-creatinine ratio≤300 mg/g),and A2 group(urine albumin-to-creatinine ratio>300 mg/g).According to the estimated glomerular filtration rate value,it was divided into G0 group(estimated glomerular filtration rate≥90 ml·min-1·1.73 m-2),G1 group(60≤estimated glomerular filtration rate<90 ml·min-1·1.73 m-2),and G2 group(estimated glomerular filtration rate<60 ml·min-1·1.73 m-2).The general data of patients,the levels of fasting plasma glucose,glycated hemoglobin,albumin,uric acid,serum cystatin C,serum creatinine,urine albumin-to-creatinine ratio,estimated glomerular filtration rate,free triiodothyronine,free thyroxine,thyroid stimulating hormone,thyroid peroxidase antibody,thyroglobulin antibody,serum creatinine,urine albumin-to-creatinine ratio,estimated glomerular filtration rate,etc.were collected.The above results were analyzed by SPSS 26.0 statistical software.Results:1.Patients in the type 2 diabetic kidney disease group had higher thyroid peroxidase antibody and thyroglobulin antibody(P<0.05).Thyroid peroxidase antibody in group A1 were higher than in group A0(P<0.05).2.The positive rates of thyroid peroxidase antibody and thyroglobulin antibody in type 2diabetic kidney disease group were higher than type 2 diabetes without diabetic kidney disease group(P<0.05).3.The result of Binary Logistic regression analysis showed that thyroid peroxidase antibody positive may be an independent risk factor of the occurrence of type 2 diabetic kidney disease(OR=9.008,95%CI(1.341-60.512)).Conclusion:1.The level and positive rate of thyroid autoantibodies were high in patients with type 2 diabetic kidney disease.2.Thyroid peroxidase antibody positive may be an independent risk factor of the occurrence of type 2 diabetic kidney disease.
