Objective: To explore the difference of efficacy and safety between domestic paclitaxel-coated balloon and bare metal stent in the treatment of lower extremity atherosclerotic occlusion,and to provide a clinical reference for the choice of stent and / or drug-coated balloon treatment for lower limb atherosclerotic occlusive disease.Methods: 79 patients with TASC? A-C femoral-popliteal atherosclerosis obliterans were collected by retrospective clinical control study.According to the different treatment methods,41 patients were divided into drug-coated balloon group(DCB group)and 38 patients were divided into metal stent group(BMS group).The preoperative,intraoperative and postoperative data of the two groups were analyzed and compared,all patients were followed up 1,3,6 and 12 months after treatment.Finally,the therapeutic effect and safety of the two groups were compared.Results: There was no statistically significant difference in the general clinical data and the degree of target lesions between the two groups(P > 0.05).There was no statistical difference in ABI between DCB group and BMS group at 1 month after operation(0.79 + 0.21 vs.0.84 + 0.20,P = 0.369).There was no statistical difference in the general rate of one stage at 12 months after operation(93.33% vs.94.74%,P=0.863).In subgroup analysis,there was no significant difference in the duration of remission of clinical symptoms after DCB treatment in ASO patients with diabetes mellitus,smoking,occlusive lesions and long-term lesions compared with those after BMS treatment.During follow-up,one severe adverse event,endovascular stent rupture,occurred in the BMS group,and no amputation or treatment-related deaths occurred in either group.Conclusion: For the treatment of femoral-popliteal atherosclerotic occlusion,domestic drug-coated balloon has good curative effect,and there is no significant difference in clinical efficacy and safety between domestic drug-coated balloon and bare metal stent.Using the drug-coated balloon for the first treatment can leave more choices for the secondary treatment when it is necessary.
