In recent years,the incidence of cardiovascular and cerebrovascular diseases has shown an increasing trend,among which stroke has attracted public attention because of its increasing incidence,high mortality and severe sequelae.In order to prevent severe disability,reduce the occurrence of a second stroke and improve the quality of life of stroke patients after discharge from hospital,this study aims at the management status of stroke patients in Nanchong City after discharge,puts forward scientific and reasonable countermeasures and suggestions,explores systematic management model,and provides further medical reference for the optimization of chronic disease management policy.A total of 26 health institutions in Nanchong City,Sichuan Province,including 9Grade IIA and above general hospitals,17 community(township)health centers,have been investigated and various aspects of stroke patients after discharge from Nanchong City have been studied by interviews and investigation.30 interviews were conducted among medical and nursing staff in general hospitals,30 interviews among community workers,30 interviews among patients' family members,and 300 questionnaires were distributed to patients.267 valid questionnaires out of 280 questionnaires have been collected.The effective return rate was 89%.The survey results are divided into two parts.First,the current situation of stroke management in Nanchong is described by relevant departments.The second is the questionnaire results of stroke patients.This study shows that the management status of stroke patients in Nanchong is poor.There are diverse factors as following: the lack of extension of hospital management service and shortage of information exchange;the insufficiency and formalization of community management;the absence of family support mechanism and management;the severity of stroke and low level of rehabilitation training knowledge of patients with depression and so on.Based on the investigation results,this paper proposes to establish a "3 + 1" management model for stroke patients after discharge from hospital,and puts forward countermeasures from four aspects.The first is to establish a continuous management system of hospital.The second is to improve the community grass-roots governance mechanism.The third is to strengthen the family institutional security.The fourth is to innovate the patient self-management mode.This model can make up the deficiency of stroke management,and it is significant to improve the stroke patients' quality of life and relieve the economic pressure of both patients' family and the society.
