Background: Hepatitis B virus(HBV)reactivation is commonly reported in blood malignancy or organ transplant patients who receiving chemotherapy or immunosuppressive therapy.There are more scholars concerned about the reactivation of HBV after liver transplantation,and there is little research or attention on HBV reactivation in allogeneic lung transplant recipients,especially in China,where HBV infection rate is high.Methods: This study included 102 patients with allogeneic lung transplants at the First affiliated Hospital of Guangzhou Medical University from November 1,2013 to December 31,2017.Collected demographic characteristics,HBV virological characteristics,immunosuppressive treatment options,follow-up time,and mortality.Grouped by HBV activation or not,statistical analysis was performed.Results data include HBV reactivation rate,HBV reactivation factors,patient survival rate and non-recurrence survival rate.Results: The median follow-up time was 39.2(33.8-44.5)months.The results showed that in 102 patients with HBsAg positive(n =8)and HBsAg negative(n =94),HBcAb positive(n = 90)and HBcAb negative(n = 12),a total of 7 patients(6.9%)developed HBV reactivation after allograft lung transplantation.The results displayed that receptor diabetes(OR = 14.112 P = 0.009)and receptor HBsAg(0R=16.529 P = 0.006)were risk factors for HBV reactivation after lung transplantation in the HBV reactivation group compared with the non-reactivation group.For patients with diabetes mellitus(p < 0.001)and HBsAg positive receptor(p=0.005),the non-recurrence survival rate after lung transplantation was lower than that of patients without the above underlying diseases.Conclusion:1?HBV reactivation can occur in patients with present and past HBV infection who receives allograft lung transplantation and was treated with immunosuppressive drugs.2?Diabetes mellitus and recipients' HBsAg+ were risk factors for HBV reactivation after lung transplantation.
