ObjectiveTo explore the effect of two double-tube drainage comfort nursing schemes in patients with enterocutaneous fistula.MethodsThe method of class experiment is adopted.According to the inclusion and exclusion criteria,86 patients with enterocutaneous fistula admitted to the gastrointestinal surgery department of a third-grade A hospital in Hunan province were selected as the research objects.The control group was a group of 43 patients admitted from May to December 2018,who were given a routine comfort care program of double cannula drainage.The experimental group consisted of 43 patients admitted from January to August,2019.The program was optimized on the basis of the conventional program,so as to form a comfortable nursing program for optimal double cannular drainage.During the study,there were 3 cases of invalid questionnaires in the control group,and 1 case withdrew from the study due to personal reasons.In the experimental group,there were 2 cases with invalid questionnaire,and 1 case withdrew from the study due to personal reasons.In the end,39 cases in the control group and 40 cases in the experimental group completed the study,with a total of 79 cases.The comfort status of patients in the two groups was assessed by the Comfort Status Scale(GCQ)at admission and discharge,and the occurrence of double cannulation drainage fluid leakage,tube blockage and tube removal during hospitalization was recorded.The differences in GCQ scores and the incidence of leakage,tube plugging and tube dropping were compared between the two groups.The measurement data between the two groups were compared using the T-test of independent samples,the Mann-Whitney U test was used when variance was uneven,the chi-square test was used for counting data,the paired sample T-test was used for comparison of measurement data before and after intervention between the two groups,and P<0.05 was considered as the standard of statistical significance.Results(1)The demographic data of two groups of patients(including gender,age,occupation and culture level,etc.),is associated with disease(skin damage and number of fistula and fistula location,etc.),GCQ scores on admission and physiological,psychological,social and environmental four dimension scores had no statistical difference(P>0.05),comparable;(2)The total GCQ score,physiological dimension,psychological dimension,social dimension and environmental dimension of the control group patients after implementing the traditional double-tube drainage comfort nursing program were 75.31±9.04 points,13.10±2.08 points,27.51±4.21 points,15.38±1.93 points,19.05±3.00 points,both higher than before the intervention,and the differences in the control group before and after the intervention were statistically significant(P<0.05);(3)The total GCQ score,physiological dimension,psychological dimension,social dimension and environmental dimension of the experimental group after implementing the optimized double-tube drainage comfort care program were 89.65+10.91 points,15.65±2.56 points,31.98±3.63 points,18.50±3.00 points,23.53±3.25 points,both higher than before the intervention,and the differences between the test groups before and after the intervention were statistically significant(P<0.05);(4)After intervention,the GCQ total score,physiological dimension,psychological dimension,social dimension and environmental dimension score of the experimental group were increased by 30.63±13.83 points,5.73±4.06 points,10.03±5.85 points,5.80±3.23 points,9.08±3.70 points,The scores are higher than the control group,the difference is statistically significant(P<0.05);(5)After the intervention,the GCQ total score,physiological dimension,psychological dimension,social dimension and environmental dimension score of the experimental group were 89.65±10.91 points,15.65±2.56 points,31.98±3.63 points,18.50±3.00 points,23.53±3.25 points;The post-control group GCQ total score,physiological dimension,psychological dimension,social dimension and environmental dimension score were 75.31±9.04 points,13.10±2.08 points,27.51±4.21 points,15.38±1.93 points,19.05±3.00 points.After intervention,the total GCQ score and the score of each dimension of the experimental group were higher than the control group,the difference was statistically significant(P<0.05);(6)During hospitalization,the incidence of plugging,leakage,and desorption in the test group was 15.0%,47.5%,and 10.0%,respectively;the incidence of plugging,leakage,and desorption in the control group was 38.5%,82.0%,and 28.2%,respectively;the incidence of plugging,leakage,and desorption in the test group was lower than that in the control group,with statistically significant differences(P<0.05).ConclusionOptimization of comfortable nursing double pipe drainage scheme is compared with the conventional double pipe drainage more comfortable nursing plan from the aspects of physical,psychological,social and environmental dimension double pipe drainage of enterocutaneous fistula patients comfort,can effectively reduce the enterocutaneous fistula patients during hospitalization in double pipe drainage discharge,the incidence of blocking pipe and pipe,improve enterocutaneous fistula patients during hospitalization and comfortable condition.
