ObjectiveThe detection of serum HBV DNA is of great significance in determining the level of viral replication for hepatitis B virus(HBV)infected patients. In this study,The correlation between HBV DNA level and hepatitis B virus e antigen(HBe Ag),liver function indicators, and the trend of HBV DNA,alanine aminotransferase(ALT)with antiviral time were explored by using hypersensitivity detection technology in the context of low viral load(?500IU/m L).Methods370 serum samples from chronic HBV-infected patients with HBV DNA virus load?500IU/m L detected by traditional fluorescence quantitative method were screened.Corresponding high-sensitivity HBV DNA levels were also detected at the same time. The 221 patients with antiviral treatment were divided into the hyper-sensitivity negative(-)group and the hyper-sensitivity positive(+)group according to the DNA level, and the liver function indexes and HBe Ag statistical differences were analyzed in the two groups.Patients with antiviral treatment were further divided into antiviral treatment for six months, one year, two years, and three years according to the treatment time.149 untreated patients with chronic HBV infection were set as the control group. The trend of high-sensitivity HBV DNA positive rate and ALT abnormal rate with antiviral time was analyzed. The trend of HBV DNA positive rate with treatment time was also analyzed in the case of abnormal serum ALT levels.ResultsThere was a statistically significant difference in the abnormal rate of ALT between the hypersensitivity(-)group and the hypersensitivity(+)group(P=0.048).The HBe Ag positive rate was also significantly different between the two groups(P<0.05).In addition, the aspartate aminotransferase(AST)abnormal rate was not significantly different between the two groups(P=0.773>0.05).The serological levels of liver function indicators were not different between the two groups(P>0.05).The positive rate of hypersensitivity HBV DNA varies with the duration of antiviral treatment, and its trend test shows a negative correlation(?2=39.823,P<0.001).With the increase of antiviral treatment time in patients with abnormal ALT,the positive rate of hypersensitivity HBV DNA gradually decreased, and the difference between the untreated group and the treated group was statistically significant(?~2=18.922,P=0.001).After using the Bonferroni method to adjust the alpha level, the two-year and three-year treatments showed statistically significant differences with the control group(P=0.002,P=0.001).ConclusionHyper-sensitivity detection is of great value in patients with low viral load HBV infection. It has a higher positive detection rate of HBV DNA than traditional fluorescent quantitative detection, which is helpful to accurately understand the level of virus replication and reduce false misjudgment caused by false negative.Combined with indicators such as ALT and HBe Ag under the background of low viral load, dynamic monitoring of the actual situation of patients will help to choose the best treatment plan.
