Research Purpose:(1)Clarify the current status of the fear of disease progression in early lung cancer patients and identify the risk factors,and then provide a certain reference for effectively preventing and reducing the occurrence of excessive fear in patients;(2)Constructing a risk prediction model for FOP based on risk factors,providing a basis for personalized nursing strategies for people with different risk levels of FOP in order to improve the quality of nursing services.Research Methods:Use the general demographic characteristics and clinically relevant data?FOP-Q-SF?PSQI?MCMQ?SSRS?GAD-7?PHQ-9 to conduct the survey of 310 patients with early lung cancer who met the inclusion exclusion criteria from January to December 2019.After the survey,the questionnaire was withdrawn on the spot and then establish a database as well as entering data for two people within 24 hours.The SPSS 21.0 software was used for statistical analysis,the general descriptive analysis was applied to general conditions,clinical data and scores of each scale(the enumeration data and grade data were expressed by rate or composition ratio,and the measurement data were expressed by sx ±).Then T-test,Analysis of variance and the Pearson correlation analysis were used to analyze the risk factors related to FOP in patients with early lung cancer.Eventually,the selected risk factors(single-factor analysis P <0.05)were incorporated and a multiple linear regression equation was used to construct a risk prediction model.Research Results:(1)The overall psychological average scores of fear of disease progression in early lung cancer patients in this study was(29.56 ± 6.53)points,of which 29.5% of patients whose average scores was(34.67±5.00)points had fear of disease progression and psychological dysfunction.The top three scores for fear of disease progression listed as follow: “Worried that the disease would progress”(3.16±0.95)points,“Worried about some big treatments during the course of the disease”(2.98±1.01)points,“I was nervous before the doctor's examination and some regular medical examinations”(2.87±1.08)points.(2)In this study,the single-factor analysis was used to screen out risk factors related to thefear progression of early lung cancer.The analysis results showed that the age,fertility situation,marital status,degree of Education,family income,work situation,family history of tumor,troubled symptoms,personality tendency,multiple nodules in the lung,PSQI(total score),sleep latency,objective support,utilization of social support,GAD-7 and PHQ-9 would affect the fear of disease progression in patients with early lung cancer.(3)Based on the results of univariate analysis,multiple linear regression analysis was used to analyze the influencing factors of fear of disease progression in patients with early lung cancer,the results showed that factors such as age,GAD-7,PHQ-9,troubled symptoms,personality tendency,and multiple nodules in the lungs were all independent factors which affected the fear of disease progression in patients with early lung cancer.(4)Build a model based on the results of multiple linear regression analysis: Y(FOP)= 31.762-0.122*age + 0.348*GAD-7 + 0.230*PHQ-9 +2.156*family history of tumor + 3.541* symptoms-1.563*characteristics + 4.298*multiple nodules or not,this regression equation could explain54.3% of the level of variability in fear disease progression.Research Conclusion:(1)Patients with early lung cancer in this research have an intermediate-to-high psychological score in the fear of disease progression,which demand attracingt the enough attention of the health care workers.(2)The fertility situation,marital status,degree of education,family income,work status,PSQI(total score),sleep latency,objective support,social support utilization are related to the progression of fear of early lung cancer patients;The younger age,high level of anxiety or depression,family history of tumor,symptomatic disturbance,more negative disposition and multiple nodules in the lung are the main influencing factors for the progression of fear of disease in early lung cancer patients.(3)The preliminary risk prediction model for the fear of disease progression in early lung cancer patients constructed in this study has a certain applicable value for identifying high-risk groups and predicting the level of FOP,and provides a new reference method for clinical evaluation.
