Objectives:To study the correlation between the level of serum Dickkopf-1(D KK1)and the degree of coronary artery stenosis in patients with corona ry atherosclerotic heart disease,so as to provide new ideas for the prev ention and treatment of coronary atherosclerotic heart disease.Methods:From March 2018 to August 2018,311 patients,including 185 males and 126 females,aged between 28-81 years,underwent coronary angiography in the Department of Cardiology,Affiliated Hospital of Chengde Medical College.The general data of patients' name,age,gender,height,weight and blood pressure were collected,biochemical indexes such as Fasting blood glucose(FBG),Triglycerides(TG),Total cholesterol(TC),High-density lipoprotein cholesterol(HDL-C),Low-density lipoprotein cholesterol(LDL-C),Uric acid(UA),Alkaline phosphatase(ALP),Calcium(Ca),25 hydroxyvitamin D(25-(OH)-D)were detected,bone turnover indexes of Osteocalcin(OC),?-collagen special sequence(?-CTX),inflammatory factor indexes of retinol binding protein 4(RBP4),plasminogen activator inhibitor(PAI-1).Before the operation,the radial artery or femoral artery puncture blood was taken and the concentration of DKK1 was measured by enzyme-linked immunosorbent assay(ELISA).According to the results of coronary angiography,subjects were divided into coronary heart disease(CHD)group(N1 = 206)and non coronary heart disease(non-CHD)group(N2 = 105).The coronary heart disease group was divided into three groups according to the genisi score: the high(n1= 67),the middle(n2 = 69),the low(n3 = 70)genisi score group.To compare the difference of the level of DKK1 among the groups,and to explore the correlation between the level of serum DKK1 and the degree of coronary artery stenosis and atherosclerotic heart disease.Results:1.Compared with non-CHD group,Age[(60.06 ± 8.68)vs(57.64 ± 10.08),P = 0.028],TG [ 1.70(1.12-2.74)vs1.38(1.02-2.0),P = 0.003],UA [313(262-380)vs 296(228-350),P = 0.033] were higher,HDL-C [1.10(0.95-1.29)vs 1.18(1.01-1.41),P = 0.004] were lower in patients with CHD.2.Compared with non-CHD group,DKK1 [5.66(4.05-7.46)vs 1.14(0.46-2.03),P < 0.05],RBP4 [11.81(7.13-17.96)vs 2.18(1.35-2.75),P < 0.05] and PAI-1 [364.85(259.21-458.73)vs 198.65(113.10-316.31),P < 0.05] were significantly higher in CHD group,OC [15.03(10.35-19.28)vs 12.53(9.98-17.56),P = 0.041] were lower in CHD group.3.Compared with the low genisi group,the high genisi group had a lower 25-(OH)-D [48.49(39.80-60.86)vs 53.85(46.84-66.10),P = 0.036].Compared with the low genisi group,the DKK1 level in the middle genisi group and the high genisi group was significantly higher[ 6.45(5.09-7.76)vs 6.13(3.90-7.57)vs 4.61(3.35-6.38),P < 0.05].Compared with the low genisi group,the RBP4 level in the middle genisi group and the high genisi group was significantly higher [13.27(9.14-16.74)vs 15.38(7.97-21.41)vs 7.51(4.89-14.62),P < 0.05].Compared with the low genisi group,the PAI-1 level in the middle genisi group and the high genisi group was significantly higher [456.06(367.68-515.90)vs 398.65(303.32-465.26)vs 256.63(198.48-326.91),P < 0.05].Compared with the low genisi group,the high genisi group had higher ?-CTX [0.26(0.16-0.38)vs 0.19(0.12-0.28),P = 0.02].4.Correlation analysis: there was a positive correlation between DKK1 and inflammatory factors RBP4,PAI-1,and the correlation coefficients(Rs)were 0.494,0.083 respectively(P < 0.05).There was no significant correlation between DKK1 and ?-CTX,OC.5.Binary logistic regression analysis: DKK1(OR=1.957,95% CI 1.476-2.954,P = 0.000),RBP4(OR= 2.638,95% CI 1.805-3.856,P = 0.000),PAI-1(OR= 1.004,95% CI 1.001-1.000,P = 0.045)are risk factors of CHD.6.Multiple ordered logistic regression analysis: the degree of coronary artery stenosis was related to the concentration of DKK1.The higher the concentration of DKK1,the more serious the degree of coronary artery stenosis,the coefficient was 0.119.Conclusions:The level of serum DKK1 was significantly higer in patients with coronary heart disease,which was positively correlated with the degree of coronary artery stenosis.DKK1 level is an independent risk factor for coronary heart disease.
