Objective:To investigate the change and clinical value of serum zinc-?2-glycoprotein level in patients with type 2 diabetic retinopathy.Methods:A total of 227 patients with type 2 diabetes mellitus were selected into this study,including 76 cases of simple diabetes patients(DM group),82 cases of non-proliferative diabetic retinopathy patients(NPDR group)and 69 cases of proliferative diabetic retinopathy patients(PDR group).meanwhile,70 healthy people as control group(NC group)were enrolled in this study.The age,sex,course of type 2 diabetes,systolic blood pressure(SBP),diastolic blood pressure(DBP),height and weight of all subjects were collected,and body mass index(BMI)was calculated.Serum creatinine(Scr),aspartate aminotransferase(AST),alanine aminotransferase(ALT),fasting postprandial glucose(FBG),total cholesterol(TC),triglyceride(TG),glycated hemoglobin A1c(Hb A1c)in all subjects and fasting plasma insulin(FINS)were determined,and the homeostasis model assessment for insulin resistance(HOMA-IR)was computed.The level of serum ZAG was detected by ELISA,the biochemical indexes were compared in the groups.Peason correlation analysis and multiple linear regression analysis were used to understand the correlation and influencing factors between ZAG and clinical indicators,and the receiver operating characteristic curve was drawn to evaluate the diagnostic value of ZAG in DR.Results:1.The serum levels of ZAG gradually decreased in the DM group,NPDR group and PDR group compared with the NC group,and the lowest level of ZAG in the PDR group(all P <0.05).2.The Pearson correlation analysis showed that serum ZAG was negatively correlated with duration of diabetes,DBP,BMI,TC,TG,FBG,Hb A1 c,FINS and HOMA-IR(r=0.15,-0.19,-0.39,-0.29,-0.47,-0.43,-0.64,-0.48,-0.60,P < 0.05),further multiple regression analysis showed that Hb A1 c,TG,HOMA-IR,BMI were independent influencing factors for ZAG(all P <0.05).3.Logistic regression analysis showed that ZAG was a protective factor for DR,and the duration of diabetes,Hb A1 c and HOMA-IR were independent risk factors for DR(all P <0.05).4.The receiver operating characteristic curve analysis showed that the sensitivity was 56.3% and the specificity was 85.5% when ZAG is 55.40?g/m L.Conclusion:The level of serum ZAG was decreased significantly in patients with diabetic retinopathy and was related to the course of disease,indicating that ZAG was closely related to the occurence and development of type 2 diabetic retinopathy,which could be used as a index for early diagnosis of diabetic retinopathy.
