Objective: This study aims to detect the expression of Interleukin 23(IL-23)and Transforming growth factor(TGF-?)in the peripheral blood and cancer tissues of patients with endometrial cancer,and analyze the relationship between the expression of IL-23 and TGF-? in the peripheral blood and cancer tissues and the clinicopathological parameters of endometrial cancer(surgical pathological staging,tissue grading,depth of myometrial infiltration,presence or absence of lymphatic metastasis)and the correlation between IL-23 and TGF-?,so as to explore the role of IL-23 and TGF-? in the occurrence and development of endometrial cancer and immune regulation,in order to provide important reference indexes for evaluating the disease progress of endometrial cancer and important reference value and theoretical basis for clinical immunotherapy of endometrial cancer.Methods: Patients with endometrial cancer admitted for surgical treatment in Gynecology Department of the Affiliated Hospital of Qingdao University from October2017 to March 2019 were selected randomly.All patients were confirmed to be with endometrial cancer by surgical pathology.60 cases of preoperative peripheral blood(the endometrial cancer group)of patients with endometrial cancer were collected,and 60 cases of paraffin-embedded tissue specimens(the endometrial cancer group)were selected at the same time.The patients were aged 38-87 years,with an average age of57.5 years.All patients did not receive radiotherapy,chemotherapy or endocrine therapy before surgery,and had no history of other tumors.The operation method was laparoscopic or laparotomic extrafascial surgery or radical panhysterectomy + double adnexa resection + pelvic cavity lymphadenectomy and/or abdominal paraaortic lymphadenectomy.In pathological staging,referring to the staging standard of the International Federation of Obstetrics and Gynecology(FIGO)in 2009,33 cases were in stage I,19 cases were in stage II and 8 cases were in stage III.Tissue grading: 22 cases of G1,27 cases of G2 and 11 cases of G3.In addition,30 cases of normal endometrial tissue(the control group)undergoing panhysterectomy due to hysteromyoma and 30 cases of the corresponding preoperative peripheral blood(the control group)were selected randomly.The expression concentrations of IL-23 and TGF-? in peripheral blood samplesof the endometrial cancer group(60 cases)and the control group(30 cases)were detected by ELISA method.The expression of IL-23 and TGF-? in endometrial cancer tissue(the endometrial cancer group,60 cases)and normal endometrial tissue(the control group,30cases)were detected by immunohistochemistry.Results:1.The expression concentration of IL-23 in peripheral blood of patients with endometrial cancer(308.704 ± 122.702 pg/ml)was significantly higher than that of the control group(179.811 ± 47.868 pg/ml)(P < 0.001),the expression concentration of IL-23 increased with the increase of endometrial cancer stage,and the difference between different stages was statistically significant(P < 0.001).At the same time,the expression concentration of IL-23 in the peripheral blood of patients with endometrial cancer with lymph node metastasis was higher than that of patients without lymph node metastasis(P< 0.05).The expression concentration of IL-23 in the peripheral blood of patients with endometrial cancer with depth of myometrial infiltration no less than 1/2 was higher than that of patients with depth of myometrial infiltration less than 1/2(P < 0.05),but the difference in expression concentrations of IL-23 in the peripheral blood of patients with endometrial cancer with different tissue grades were not statistically significant(P >0.05).2.The expression concentration of TGF-? in the peripheral blood of patients with endometrial cancer(2750.838 ± 721.447 pg/ml)was significantly higher than that of the control group(1676.027 ± 678.759 pg/ml),and the expression concentration of TGF-?increased in turn with the increase of endometrial cancer stages,and the differences between different stages were statistically significant(P < 0.001).At the same time,the expression concentration of TGF-? in the peripheral blood of patients with endometrial cancer with lymph node metastasis was significantly higher than that of patients without lymph node metastasis(P < 0.001).The expression concentration of TGF-? in the peripheral blood of patients with endometrial cancer with depth of myometrial infiltration no less than 1/2 was higher than that of patients with depth of myometrial infiltration less than 1/2,with statistically significant difference(P < 0.05).The expression concentrations of TGF-? in the peripheral blood of patients with endometrial cancer with high differentiation,medium differentiation and low differentiation increased in turn,with statistically significant difference(P < 0.05).3.The positive expression rates of IL-23 in endometrial cancer tissue(the endometrial cancer group)and normal endometrial tissue(the control group)were78.33% and 26.67% respectively,with statistically significant difference(P < 0.05).The positive expression rates of TGF-? in endometrial cancer tissue(the endometrial cancer group)and normal endometrial tissue(the control group)were 81.67% and 23.33%respectively,with statistically significant difference(P < 0.05).4.The expressions of IL-23 and TGF-? in peripheral blood of patients with endometrial cancer were correlated(r = 0.39,P < 0.01),and there was a positive correlation between them.Conclusion:1.The expression levels of IL-23 and TGF-? in the peripheral blood of patients with endometrial cancer were both high,and the expression levels of IL-23 and TGF-?increased gradually as the grades of clinicopathological parameters(surgical pathological staging,tissue differentiation,depth of myometrial infiltration,lymphatic metastasis)of patients with endometrial cancer got worse.Besides,the expressions of IL-23 and TGF-?also showed a significant increase in endometrial cancer tissue.It indicated that IL-23 and TGF-? might participate in the process of occurrence and development of endometrial cancer,and at the same time,it also indicated that there might be an immune imbalance in the body of patients with endometrial cancer,and both IL-23 and TGF-? participated in the immune process of endometrial cancer.2.The high expressions of IL-23 and TGF-? in the peripheral blood of patients with endometrial cancer were positively correlated,indicating that they might participate in the occurrence and development of endometrial cancer through certain interaction.
