Objective: The clinical data of colorectal cancer patients were analyzed.The levels of CEA,CA199 and CA125,the status of peritoneal metastasis of colorectal cancer patients before operation,and the pathological data after operation were collected.The diagnostic efficacy of various serum tumor markers for peritoneal metastasis of colorectal cancer was compared with that of various serum tumor markers Prediction of tumor markers in peritoneal metastasis of colorectal cancer.Methods: The data of patients who underwent colorectal surgery in gastrointestinal surgery of Affiliated Hospital of Qingdao University from January 2016 to December 2018 were analyzed retrospectively.From January 2016 to December 2018,a total of 487 colorectal cancer patients were selected from the first clinical medical college affiliated to Qingdao University.Among them,251 were male and 236 were female According to preoperative imaging and surgical confirmation,patients undergoing colorectal cancer surgery were divided into two groups: group without peritoneal metastasis and group with peritoneal metastasis;the levels of CEA,CA199,CA125,etc.of preoperative serum tumor markers,peritoneal metastasis of colorectal cancer during operation and postoperative pathological data were collected,and the diagnosis of peritoneal metastasis of colorectal cancer by various serum tumor markers was compared To investigate the predictive effect of various serum tumor markers on peritoneal metastasis of colorectal cancer.Results: In the 487 patients with colorectal cancer in this study,patients with welldifferentiated adenocarcinoma did not develop peritoneal metastasis.The probability of peritoneal metastasis in moderately-differentiated adenocarcinoma was 13.1%.The probability of peritoneal metastasis is 26.3% ? 71.2%.The higher the malignant degree of pathological differentiation,the more likely it is peritoneal metastasis(2 = 750.402,P <0.001);serum tumor marker CEA(Z = 4.316,P <0.001),CA199(Z = 4.306,P <0.001),CA125(Z = 5.385,P <0.001),the more obvious the increase in the level,the greater the probability of peritoneal metastasis;when these three indicators increase jointly(especially CEA + CA125),CA199 + CA125)or three increases in the incidence of peritoneal metastasis is higher.The probability of peritoneal metastasis when the tumor markers are combined with the degree of clinical pathological differentiation is more obvious than the probability of peritoneal metastasis when each index is detected separately;serum CEA,CA199,and CA125 metastasis in colorectal cancer Among the auxiliary diagnoses,CA125(AUC 0.716,P value 0.001,threshold 17.12)is the most sensitive,100%,and the area under the curve is 0.716,which is significantly higher than the other two.The specificity of CA125 is close to CEA(AUC 0.637,P value 0.042,threshold 6.53),which is lower than CA199(AUC 0.667,P value 0.013,threshold 38.57).CA199 has the highest specificity of 91%,but the lowest sensitivity(45%).Conclusions: The degree of tumor differentiation and the level of serum tumor markers in patients with colorectal cancer are related to peritoneal metastasis.The more malignant the tumor,the greater the probability of peritoneal metastasis of colorectal cancer;the higher the detection value of serum tumor markers,the higher the probability of peritoneal metastasis of colorectal cancer;several commonly used serum tumor markers,CA125 is the most sensitive and effective in the auxiliary diagnosis of peritoneal metastasis of colorectal cancer,CA199 The sensitivity of combined two or more serum tumor markers was higher than that of single use.
