Background: Pemphigus is a chronic and recurrent autoimmune skin disease.It is clinical manifestation is on the basis of normal skin or mucous membranes and erythema,there are loose blisters,bullae,thin blister wall,easy to rupture,and often positive for Nissl sign.The characteristics of histopathology are acanthosis and intraepidermal blisters.In recent years,studies have shown that chronic inflammatory diseases have dyslipidemia.These diseases include psoriasis,systemic lupus erythematosus,AIDS,etc.Pemphigus is also a chronic inflammatory disease,but there is little research on serum lipids in pemphigus patients at home and abroad.This article analyzes whether pemphigus patients have dyslipidemia by collecting relevant data.Objective: In this paper,we collected data on serum lipid metabolism in patients with pemphigus,including TG,HDL,LDL,apoB,apoA1,Lp(?),FFA,BMI,to study the disorder of lipid metabolism in patients with pemphigus,to analyze the characteristics of serum lipid changes in patients with different types of pemphigus,to determine whether pemphigus has dyslipidemia,so as to provide clinical application value for the further treatment of pemphigus.Methods: Collecting inpatients diagnosed with pemphigus in the dermatology department of the Affiliated Hospital of Qingdao University from 2014 to 2018 by Qingdao University Affiliated Hospital Yidu Cloud Database.However,because the number of patients with special types of pemphigus is small,which affects the results of serum lipid analysis,the special types are excluded.In addition,the patients who were treated with hormones before admission were excluded,and the remaining patients after the removal were classified as disease groups.According to the characteristics of skin lesions and tissue immunopathology,the disease group is divided into group A and group B,that is,the common type and proliferative type are group A,and the deciduous type and erythema type are group B.In the same period,the healthy people in our hospital's medical examination center were used as the control group.Exclusion criteria :(1)Combining patients with hypertension,hyperlipidemia,and diabetes;(2)Patients with severe endocrine diseases such as Cushing syndrome;(3)Patients who did not use drugs that have an effect on serum lipid levels within four weeks before admission,such as glucocorticoids,avitamin A,statins,fibrates,etc.Recording the demographic indicators,BMI,and serum lipid metabolism-related indicators(TG,HDL,LDL,apoB,apoA1,Lp(?)).The method of detecting serum lipids is to collect patients' fasting venous blood in the morning,and use the automatic biochemical analyzer of our laboratory to quantitatively analyze and test fasting blood samples.All data are analyzed using SPSS17.0,the measurement data that conforms to the normal distribution in the test results are expressed as x ± s,and the data with positive skew distribution are expressed as M(P25–P75).The data of the normal distribution were compared by two samples using the t test of independent samples,and the comparison of multiple groups was by one-way analysis of variance;the data of non-normal distributions were tested by the rank sum test.Results: In this study,a total of 99 pemphigus patients were collected.According to the above inclusion criteria,43 patients were finally included in the disease group,including21 males and 22 females,with an average age of(49.2 ± 7.6)years,including 27 patients in group A 16 cases in group B,accounting for 62.8% and 37.2% of the total number of disease groups,respectively.In the same period,50 patients who passed the health examination in our hospital were used as the control group,including 25 males and 25 females,with an average age of(48.8 ± 7.5)years old.The levels of serum LDL,apoB and FFA in the disease group were higher than those in the control group;TG,HDL,apoA1,Lp(?)and BMI were not significantly different between the two groups.Serum apoB and FFA levels in group A were higher than those in control group,but there were no significant differences between groups A and B,and between group B and control group.There was no significant difference in HDL,apoA1 and Lp(?)between group A and control group,between group A and group B,and between group B and control group.Conclusion: Patients with pemphigus have abnormal serum lipid metabolism,and their disorder may be related to the type of disease.Abnormal serum lipid metabolism can be considered as an important part of the pathological process of pemphigus.The study on the correlation between dyslipidemia and pemphigus has certain significance for further understanding of the pathogenesis and progress of pemphigus and its understanding.
