OBJECTIVE:In order to achieve accurate and minimally invasive root canal treatment and enhance the fracture resistance of tooth tissue after root canal treatment,this study explored digital guided mediated lower pulp treatment and compared it with conventional root canal treatment to provide a more favorable treatment method for clinical practice.METHODS:In this study,a number of fresh mandibular first permanent molars were extracted within 3 months,and 40 teeth were screened by CBCT scan.They were randomly divided into control group and experimental group,with 20 in each group.Then,the control group used conventional root canal treatment for opening the pulp chamber,while the experimental group used Endo root canal treatment software to design the access cavities,and made personalized guide plate with the help of 3D printer.With the help of guide plate,accurate and minimally invasive the access cavity opening method was adopted.In both the control group and the experimental group,TF nickel-titanium file was used for root canal preparation and thermoplasticized gutta-percha technique for root canal filling to complete the subsequent root canal treatment.In the operation process,the difference of cavity-opening operation consumption time and root canal preparation time between the two groups was recorded and compared,and the proportion of the open pore area to the total occlusal surface area of the two groups was calculated by using AutoCAD software.After the restoration of all 40 teeth by resin filling and biomimetic treatment of materials such as silicone rubber and self-condensing plastic,the loading test was carried out on the teeth by universal testing machine,and the results were recorded until the teeth were cracked,so as to compare the fracture strength of the two groups.In this study,SPSS 24.0 software package was used for statistical analysis of data.RESULTS:In the control group,the time required for opening the pulp chamber was(1.85±0.05)min,the open pore area was(9.18±0.48)% of the total occlusal area,and the load of the tooth tissue was(1.48±0.07)kN.In the experimental group,the time required was(0.72±0.10)min,the open pore area was(3.53±0.13)% of the total occlusal area,and the load of the tooth tissue was(1.81±0.03)kN.The higher the loading value,the stronger the fracture resistance of the tooth tissue.Compared with traditional root canal treatment,digital guided plate mediated minimally invasive root canal treatment had the advantages of short time,small access cavity and strong fracture resistance of tooth tissue.The difference between the two groups was significant(P<0.01).CONCLUSION:Digital guided plate mediated accurate minimally invasive root canal treatment can reduce the occlusal area,shorten the operation time beside the chair,retain more healthy tooth tissues,enhance the fracture resistance of tooth tissue after root canal treatment,and improve the retention rate of affected teeth.
