Objective: Epidermal growth factor receptor(EGFR)mutation is one of the major therapeutic targets of non-small cell lung cancer(NSCLC).It is found that EGFR mutant NSCLC has unique biological behavior,but the mechanism remains to be explored.Chemokine receptor 4(CXCR4),a member of the Chemokine receptor family,is involved in the development of various malignant tumors and with functions of promoting the growth,proliferation and migration of tumor cells.In this study,the clinicopathological characteristics of EGFR mutant NSCLC will be analyzed.To explore the expression of CXCR4 protein in different EGFR states,we detected the expression of CXCR4 protein in NSCLC by immunohistochemistry.This study may reveal the biological behavior of EGFR mutation,it also provides a new direction for the treatment of tyrosine-kinase inhibitor(TkIs)resistant NSCLC.Methods: 163 cases of NSCLC with pathological diagnosis and EGFR detection of the ShengJing Hospital were selected.We analyzed the correlation between EGFR mutation and clinicopathological characteristics in NSCLC patients.The expression of CXCR4 protein in NSCLC was compared with that in normal lung tissues by immunohistochemistry.The relationship between CXCR4 protein and clinicopathological characteristics of NSCLC,and the expression of CXCR4 protein in different EGFR mutant subtypes were also investigated.Kaplan-Meier was used to analyze the significance of EGFR mutation and CXCR4 protein in the prognosis of NSCLC.Results:(1)In this study,EGFR mutations were most common in younger,female NSCLC patients without smoking history,and the frequency of EGFR mutations in patients with high-differentiated adenocarcinoma was higher(P < 0.05).EGFR mutation in T1-2 stage and M1 stage NSCLC was more common(P < 0.05).(2)CXCR4 protein is highly expressed in NSCLC and especially in patients with lymph node metastasis and late TNM stage,which is an indicator of poor prognosis(P < 0.05).(3)The expression of CXCR4 protein in EGFR mutant NSCLC was higher than that in the wild type,and the expression of CXCR4 protein in the 21-L858 R mutant NSCLC was higher than thatin the wild type and 19-DEL type(P < 0.05).(4)The overall survival of NSCLC patients with high expression of EGFR wild-type NSCLC and CXCR4 protein was lower,and both were independent risk factors affecting the prognosis of NSCLC patients(P < 0.05).Conclusion:(1)In NSCLC,EGFR mutations are more common in women,younger patients,no smoking history,and highly differentiated adenocarcinoma.The prognosis of patients with EGFR wild-type NSCLC is poor.(2)CXCR4 protein is highly expressed in NSCLC,which is mostly seen in patients with lymph node metastasis and late TNM stage.It is an indicator of poor prognosis.(3)The expression of CXCR4 protein in EGFR mutant NSCLC was higher than that of the wild type,and the expression of 21-L858 R mutant was higher than that of other mutant subtypes.The biological characteristics of EGFR mutant NSCLC may be related to the high expression of CXCR4 protein,and CXCR4 may be a therapeutic target of EGFR mutant NSCLC.
