Objective:This study on the basis of hospital group,use cases crossover design,through the comparison with six months to one year in the first two paragraphs of recurrence in patients with sleep tim difference,difference and other risk factors,and thus may affect the risk of ischemic stroke recurrence factors,to provide basis for prevention of recurrence of ischemic stroke.Methos:1.Consecutive hospitalized patients diagnosed with recurrent ischemic stroke on July 1,2018 and September 30,2019 were collected in a neurology ward of a hospital in Liaoning province.After screening by inclusion and exclusion criteria,a total of 328 subjects were included-2.A unified design of the investigation form was adopted to conduct the investigation by on-site inquiry and retrieval of medical record data.Research contents include:basic information,history recurrence of ischemic stroke,ischemic stroke and other cardiovascular diseases history and family history,with the onset of ischemic stroke may be related to the behavior of the factors,Pittsburgh sleep quality index scale(PSQI)research obj ect nearly six months sleep condition,the object of study on the factors of recurrence,the first half and half a year to a year.3.Epidata software was used to establish the database and double-input the data.SPSS 22.0 sofware was used for statistical analysis.The baseline data and sleep-related conditions of the study subjects were dscribed,and the chi-square test of paired data and Wilcoxon rank sum test were used to analyze the differences of various factors in the first half year and the second half to the first year of the study subjects' recurrence,and the subgroup analysis was compared by gender and the number of recurrencce.A conditional logistic regression model was used to analyze the factors associated with therecurrence of ischemic stroke.Results:Total of 328 subjects were included in the study,including 208 males(63.4%)and 120 females(36.6%).The average age of the subjects was 59.96±9.72 years.Description 1.The basic situation of the research object,in all the research object a BMI of 28 or 58 cases(17.7%)?current smoking,126 cases(38.4%),112 cases(34.7%)and drinking at the present stage almost 199 cases(60.75),do not exercise object 197 cases(60.1%)to the research on the history of high blood pressure,diabetes,history of 57 cases(17.4%,recurrence rate as the research object of 1 199 cases(607%).recurrence of parts for the same side of the object of study,188 cases(57.3%),174(53.0%)subjects had good sleep quality,71(21.6%)subjects slept less than or equal to 6 hours,and 112(34.1%)subjects slept more than or equal to 9 hours.2.The results of case crossover design showed that there were statistically significant differences in waking early or waking easily,difficulty in sleeping after waking and whether to take drugs for chronic diseases;Multivariate logistic regression found that the risk of ischemic stroke recurrence was 1.592 times(OR=1.592.95%ci=1.039-2.438)for those who woke up early OR who woke up easily occasionally(OR=1.592,95%ci=1.039-2.438).The risk of recurrent ischemic stroke was 1.362 times higher in those who woke up frequently OR who woke up easily(OR=1.362,95%ci=1.073-1.728).The risk of recurrent ischemic stroke was 1.462 times higher in those who had difficulty falling asleep after frequent wakefulness than in those who had difficulty falling asleep after almost no wakefulness(OR=1.462,95%CI=1.16-1.85).3.Taking gender as the subgroup,the difference in early awakening or easy awakening was only found to be statistically significant in males(P<0.05).There were no statistically significant differences among women.4.The frequency of recurrence was taken as the subgroup,and there were statistically significant differences in waking up early or waking up easily and taking drugs for chronic diseases(P<0.05).There were statistically significant differences in wake up early or wake up easily and pickles consumption between the two groups(P<0.05).Conclusions:A multivariate analysis found that the presence of insomnia symptoms such as early awakening,easy awakening,or difficulty in falling asleep during the first six months of a relapse may increase the risk of recurrence of ischemic stroke.In the subgroup analysis,it was found that the number of men who often woke up early or easily woke up increased in the first six months after the recurrence of ischemic stroke.In the first half year before the recurrence:the he mumber of people who often woke up early or easily woke up increased,while the number of people who took drugs for chronic diseases decreased.In the group with two or more recurrences:the number of people who often woke up early or easily woke up and the number of people who ate pickles increased.
