Objective: This study aimed to compare the complications of the donor site of supraversus subfascially harvested anterolateral thigh perforator flaps,in order to provide a better choice for the clinical practice.Method: We searched Pub Med,Web of Science,EMBASE,the Chinese Bio Medical Literature Database(CBM)and the Cochrane Library until December 31,2019,to identify studies that compared complications at the donor site of the supra-versus subfascially harvested anterolateral thigh perforator flaps. The rate of poor healing in the donor site, dysfunction at the donor site and sensory functions at the donor site were evaluated.Result: Six studies with a total of 486 patients were included in our analysis. In the study of sensory abnormalities in the donor site,152 cases were included in 3 studies, and the results showed that there was no statistical difference between the two methods(P >0.05).In the study of poor healing in the donor site,486 cases were included in 6 studies, and the results showed that SPF was superior to SBF(RR = 0.392,95% CI: 0.186-0.829,P = 0.014),the results were significantly different. In terms of dysfunction in the donor site,380 cases were included in 4 studies, and the results showed that SPF was superior to SBF(RR = 0.176,95% CI: 0.059-0.528,P = 0.002),the results were significantly different. Conclusion: Our meta analysis shows that the method of harvested anterolateral thigh perforator flaps can reduce the complications of the donor site, and the healing of the donor site and functional recovery of the donor site are better than the subfascial method.Clinically,we respect the suprafascial method.
