Objective: To investigate whether voxel-based morphological measurement(VBM)can play an auxiliary role in the surgical treatment of medically refractory temporal lobe epilepsy,and to analyze the factors influencing the prognosis of surgery for refractory temporal lobe epilepsy.Methods: The clinical data of 36 patients with drug-resistant temporal lobe epilepsy who underwent surgical treatment in the department of functional neurosurgery of Shengjing hospital affiliated to China medical university from January 2013 to January 2019 were retrospectively analyzed.All patients were followed up for 1 year,and the follow-up results were graded by Engel.VBM method was applied for analyzing the changes in the volume and density of gray matter in the whole brain.Combined with the results of video electroencephalography(VEEG),MRI,PET and postoperative pathology,the role of multimodality image post-processing technology was discussed.At the same time,different pathological classification,whether MRI/PET results were positive,whether the discharge in VEEG results was limited,whether VBM was consistent with MRI/PET and VEEG results and other relevant factors that might affect the surgical prognosis were analyzed.Statistical method Fisher's accurate test was used.Results: Whether the results of VBM,MRI/PET and VEEG were consistent or not had a statistically significant effect on the surgical prognosis(P<0.05),but no statistically significant effect on the FCD classification of pathological results(P>0.05).When MRI/PET results were negative,the consistency between VBM results and VEEG results had a statistically significant effect on surgical prognosis(P<0.05).The limitation of discharge in the VEEG results was an independent factor influencing the surgical prognosis(P<0.05),while the positive MRI/PET results and FCD classification of pathological results were not independent factors influencing the surgical prognosis(P>0.05).Conclusion: Multimodality image post-processing technique based on voxel morphological measurement(VBM)can guide the surgical treatment and prognosis of drug-resistant temporal lobe epilepsy.The surgical prognosis of refractory temporal lobe epilepsy is closely related to the results of video electroencephalography.
