Clinical Efficacy Of The Combination Of Gabapentin And Anti-histamine Drugs In The Treatment Of Refractory Pruritus

Objective:By observing the changes in the degree of skin itching,the quality of life,the sleeping time and the adverse reactions of patients with refractory pruritus before and after treatment by the combination of Gabapentin and Anti-histamine Drugs,to explore the clinical efficacy and safety of Gabapentin in the treatment of pruritus skin diseases,which provided some theoretical basis for clinical application of Gabapentin.Methods:The self-contrast clinical observation method was used.All the 25 cases were recruited from patients in the dermatology department of the First Affiliated Hospital of China Medical University from December 2017 to January 2020.The patients who met the criteria for refractory pruritus and Still suffering from refractory pruritus(modified DUO's pruritus score decreased by less than 8 points)after two weeks of treatment with two or more anti-histamines were in the Before adding medicine group,and all 25patients were treated with Gabapentin combined with primary anti-histamine drugs for two weeks as the After adding medicine group.Relevant indicators were recorded before treatment,two weeks after with antihistamines alone and two weeks after combined with gabapentin:including VAS(Visual Analogue Scale),modified DUO pruritus score,sleep time,and Dermatology Quality of Life Index Questionnaire(DLQI).The effective cases on the first,fourth,seventh and fourteenth day after the combination of gabapentin were recorded.At the same time,the adverse reactions during medication were recorded,and the clinical safety of Gabapentin was evaluated.Results:1.After treatment,compared with the Before adding medicine group,the VAS,the modified DUO's pruritus,and the DLQI score in the After adding medicine group were significantly lower and the sleep time was higher.The difference was statistically significant,(P<0.05);2.The effective time of 25 patients was 1-4 days.On the first day,8 patients'pruritus was relieved,the total effective rate was 32.00%.On the fourth,seventh and fourteenth day,23 patients'pruritus was controlled,the total effective rate was 92.00%.After 14 days of treatment,all the 25 patients in the Before adding medicine group were ineffective,the total effective rate was 0.00%.The total effective rate of the After adding medicine group was significantly higher,and the difference between the two groups was statistically significant(?2=42.593,P<0.05);3.In the After adding medicine group,there were 2 cases of dizziness and 1 cases of nausea occurred during treatment,the incidence of adverse reactions was 16.00%.In the Before adding medicine group,2cases of dizziness and 1 of nausea occurred during treatment,the incidence of adverse reactions was 12.00%.There was no significant difference.(?~2=0.194,P>0.05).Conclusion:Gabapentin combined with anti-histamines can significantly relieve itching in patients with refractory pruritus,improve patients'quality of life,and increase their sleep time.Compared with the use of multiple antihistamines alone,the incidence of adverse reactions has not increased,the safety of Gabapentin is high and with good tolerance,it has certain value in clinical medication.
