Infection Rate Of Helicobacter Pylori In Adults And Comparison Of 14-day Bismuth-containing Quadruple Therapy For Helicobacter Pylori Eradication With Different Antibiotic Combinations In Shenyang City

Objective:To explore the infection rate of Helicobacter pylori?HP?in adults and compare the eradication rate,adverse reactions and compliance of the two 14-day bismuth-containing quadruple therapies as the first-line HP eradication programs in Shenyang City.Methods:This study used case collection method.We collected all patients who went to outpatient and physical examination center of the First Affiliated Hospital of China Medical University for¹³C breath test from January 2015 to December 2018,and selected 222 eligible HP-infected patients from March 2017 to October 2018 and treated with either of the following two programs:?1?EBLA group:esomeprazole?20 mg,twice a day?,bismuth?200 mg,twice a day?,levofloxacin?200 mg,twice a day?and amoxicillin?1g,twice a day?,111 cases in total;?2?EBCA group:esomeprazole?20mg,twice a day?,bismuth?200 mg,twice a day?,clarithromycin?500 mg,twice a day?and amoxicillin?1g,twice a day?,111 cases in total.The patients in both groups were given medicines for 14 days,and the ¹³C breath test was performed after 4 weeks of withdrawal.Results:From 2015 to 2018,the HP infection rates of adults were 40.31%,43.73%,35.20%and 36.40%in Shenyang City respectively.The infection rates of male and female were 41.76%and 37.23%respectively,the difference was statistically significant?P<0.05?.The infection rate was 29.08%for people aged 10 to 20,34.41%for people aged 20 to30,38.70%for people aged 30 to 40,38.71%for people aged 40 to 50,40.07%for people aged 50 to 60,40.69%for people aged 60 to 70,41.82%for people aged 70 to 80.The eradication rate of EBLA group was 87.34%?95%CI 80.9%-93.9%?,and that of EBCA group was 83.33%?95%CI 76%-90.7%?.There was no significant difference between the two groups?P=0.413?.The incidence of adverse reactions was10.8%in EBLA group and 26.13%in EBCA group.The difference was statistically significant?P=0.003?.The compliance of patients in EBLA group was 99.10%,and that of EBCA group was 98.20%.The difference was not statistically significant?P=0.561?.Conclusion:The infection rate of HP in Shenyang City has fluctuated from 35%to 44%in recent years,and the infection rate of male was higher than that of female.The lowest and highest age groups of the infection rate were in the age of 10 to 20 and 70 to 80respectively.The infection rate increased with age.Both EBLA program and EBCA program could achieve good eradication rate and compliance in Shenyang City.The incidence of adverse reactions in EBLA group was lower than that in EBCA group.
