Objective To explore the expression of Rac1 relations with laryngeal cancer clinical pathologic factors,and to analyze the survival rate,and to study the Rac1 role in the occurrence,infiltration and metastasis of laryngeal cancer,and to clear Rac1 application value in the diagnosis and prognosis of laryngeal cancer,we detected the expression of Ras-related C3 botulinum substrate 1 in laryngeal squamous cell carcinoma(LSCC),by measuring Rac1 protein expression in LSCC tissues and the adjacent normal laryngeal tissues.Methods The clinical data of 63 LSCC patients after operation were retrospectively analyzed,including gender,age,drinking history,smoking index,tumor size,tumor site,histological grade,lymph node metastasis,T stage,TNM stage,whether to have radiotherapy and whether to have recurrence.The expression of Rac1 in laryngeal cancer and paracancer tissues was determined by immunohistochemistry.The results were compared to investigate the relationship between the expression of Rac1 and the clinicopathological factors of laryngeal cancer.TNM stage was based on UICC laryngeal cancer staging criteria in 2002.Paracarcinoma nomal tissue specimens were obtained from laryngeal body specimens undergoing total laryngectomy for laryngeal cancer.The tissue taken was visually observed at least 2cm from the edge of the tumor,and no cancerous tissue was found under the microscope.Results(1)The positive expression rate of Rac1 protein in LSCC cancer tissues was 73.02%,and the positive expression rate in paracancer nomal tissues was 10.00%,There was a statistically significant difference between laryngeal cancer tissues and paracancer nomal tissues(P<0.01);(2)The expression level of Rac1 in LSCC cancer tissues was correlated with tumor size,smoking index,histological grade,T stage,clinical stage and whether there was lymph node metastasis(P<0.05);(3)The results of single-factor survival analysis showed that the high expression of Rac1 was related to the low survival rate of laryngeal cancer patients,and the prognosis of laryngeal cancer became worse with the increase of the expression level;(4)The results of multi-factor survival analysis showed that Rac1,histological grade,clinical stage,lymph node metastasis,tumor size and recurrence were independent indicators of the prognosis of laryngeal cancer.The overall 3-and 5-year survival rates were 81.91% and 72.14% respectively.Conclusions(1)Compared with normal adjacent tissues,the positive rate of Rac1 expression in laryngeal cancer tissues was significantly increased,suggesting that Rac1 may be involved in the occurrence and development of laryngeal cancer;(2)The high expression of Rac1 in laryngeal cancer was correlated with histological grade,T stage,TNM stage,lymph node metastasis and other clinicopathological parameters,suggesting that there might be some mechanism between them;(3)Survival analysis showed that Rac1 was related to the low survival rate and poor prognosis of laryngeal cancer patients,and might be an independent risk factor affecting the prognosis of laryngeal cancer;(4)With the decrease of histological differentiation of laryngeal cancer,the increase of TNM stage and the occurrence of lymph node metastasis,the positive rate of Rac1 increased successively,which was helpful for the early diagnosis and prognosis assessment of laryngeal cancer,suggesting that Rac1 could be used as one of the biological targets of laryngeal cancer.
