Study On The Influence Of Physician' Empathic Ability On Shared Decision Making

Background: Shared decision making has been proposed and studied for more than 40 years in foreign countries.It is a common clinical decision making model in the west.In recent years,Chinese researchers gradually attach importance to the concept of shared decision making and try to explore the feasibility of shared decision making in clinical practice.Therefore,it is an urgent problem to construct and promote the shared decision making model in line with China's medical environment.Shared decision making is an appropriate decision based on empathy,and the relationship between physician's empathic ability and shared decision making needs further discussion.Purpose:Based on physician perspective,using questionnaire survey and data analysis,understand the current physician empathic ability and the present situation of shared decision making,analyze the demographic sociological factors that affect physician's empathic ability and shared decision making,discuss the relationship between physician's empathic ability and shared decision making,to bridge the psychological differences between physicians and patients and diverging decision-making,to provide empirical reference basis and starting point for implementing effective shared decision making.Methods:(1)Literature analysis: By looking up domestic and foreign documents,combing and summing up the practical dilemma of sharing decision making,to lay a theoretical foundation for exploring the relationship between physician's empathic ability and sharing decision-making.(2)Questionnaire Survey: Using the convenience sampling method and the paper questionnaire and the network questionnaire were used to conduct the questionnaire survey on the clinicians in the first,second and third grade hospitals in Guangzhou.Jefferson scale of physician empathy and Interpersonal Reactivity Index were used to evaluate physician's empathic ability,and Shared Decision Making Physician Version scale was used to investigate physician's participation in shared decision making.(3)Statistic analysis: SPSS version 22 was used for analysis the influence of physician's empathic ability on shared decision making.The analysis methods included two independent sample t test,one-way analysis of variance,Pearson correlation analysis,and multiple linear regression analysis.Results :(1)The influence of demographic sociological characteristics of physicians on empathic ability.The cognitive empathy and emotional empathy of physicians in different age,working years,educational background,professional title,hospital type,hospital grade and departments showed statistical differences(P<0.05).There was significant difference in imagination of physicians with different working years and departments(P<0.01).(2)The influence of demographic sociological characteristics of physicians on shared decision making.There was a statistical difference in sharing decision making among physicians of different hospital types(P<0.01).Physicians in general hospitals practice shared decision making better than physicians in specialized hospitals.(3)The influence of physician's empathic ability on shared decision making.The cognitive empathy of physicians is significant positively correlated with the information factor and treatment factor in shared decision making scale(P<0.05),of which perspective taking has a positive effect on the information factor and the treatment factor.The imagination is significant positively correlated with the information factor and treatment factor in shared decision making scale(P<0.01),and has a positive effect on information factor.The empathy concern in emotional empathy is significant positively correlated with the information factor and treatment factor in shared decision making scale(P<0.05).The personal distress in emotional empathy is significant negatively correlated with the information factor in shared decision making scale(P<0.05),and negatively affects the information factor.Discussion: Physician's empathic ability has an influence on shared decision making.Different empathic abilities play different roles in shared decision making.The cognitive empathy ability and the imagination have positive influence on shared decision making.Personal distress in emotional empathy ability has a negative influence on shared decision making.The following Suggestions are put forward:(1)attach importance to the cultivation of physicians' cognitive and empathic ability;(2)strengthen the emotional management of physicians in a targeted way;(3)Improve physicians' medical imagination.
