| Objective:By comparing the difference of immediate hemostasis rate,detection rate of responsible vessels,survival rate without hemoptysis and risk factors of hemoptysis recurrence between preoperative multislice spiral CT angiography and non-preoperative multislice spiral CT angiography in patients with hemoptysis,to explore the effect of thoracic multislice spiral CT angiography before bronchial artery embolization on the recurrence of hemoptysis.Methods:The clinical data of hospitalized patients with hemoptysis in the Department of Respiratory and critical Medicine,Zhang jia jie people’s Hospital affiliated to Hunan normal University from May 2017 to May 2019 were collected.Finally,67 patients with hemoptysis who received bronchial artery embolization were included.After one-year follow-up,the immediate hemostasis rate,the detection rate of responsible vessels,the survival rate of hemoptysis and the risk factors of hemoptysis recurrence in the two groups were analyzed.Results:1.The overall immediate hemostasis rate of patients with hemoptysis in the two groups was 85.07%(57/67),MSCTA group 88.57%(31/35)and 81.25%(26/32)in the control group,there was no difference in the immediate hemostasis rate between the two groups(X2=0.706,P=0.401).2.The number of responsible vessels in the MSCTA group was more thanthat in the control group(1423.5vs854.50 [rank sum],P=0.003),and MSCTA was more favorable for the detection of non-bronchial artery system vessels under DSA(1372.0vs906.00 [rank sum],P=0.018).3.The 7-day hemoptysis-free survival rate in the MSCTA group was significantly better than that in the control group(80.0% [28/35] vs53.1% [17/32],P=0.011).4.MSCTA before bronchial artery embolization is an independent risk factor for recurrence of hemoptysis,and the MSCTA group is 3.5 times better than the control group.(OR = 3.529,95% confidence interval: 1.198—10.401,P=0.022).Conclusion:1.Multi-slice spiral CT angiography before bronchial artery embolization had no significant effect on the rate of immediate hemostasis.2.Multi-slice spiral CT angiography before bronchial artery embolization is helpful to detect the responsible vessels of hemoptysis and non-bronchial artery system.3.Multi-slice spiral CT angiography before bronchial artery embolization can improve the 7-day survival rate of hemoptysis patients without hemoptysis.4.Multi-slice spiral CT angiography before bronchial artery embolization is an independent risk factor for recurrence of hemoptysis.Routine chest MSCTA examination can be recommended for patients with hemoptysis. |
