Objective To evaluate the clinical effect of modified coronally advanced flap(MCAF)combined with platelet rich fibrin membrane(PRF)or subepithelial connective tissue grafts(SCTG)in the treatment of Miller ? gingival recession through a randomized controlled study,so as to provided a theoretical basis for better clinical effect of mucogingival surgery.Methods From June 2018 to November 2019,patients in the Department of periodontology of the Affiliated Stomatology Hospital of Anhui Medical University were selected.For 12 patients with Miller ? gingival recession were treated with periodontal plastic surgery,i.e.modified coronally advanced flap+platelet rich fibrin membrane(MCAF+PRF membrane)and modified coronally advanced flap+subepithelial connective tissue grafts(MCAF+SCTG).Among them,6 patients(15teeth)were included in the test group(MCAF+PRF membrane group)and 6 patients(15teeth)in the control group(MCAF+SCTG group).The gingival recession depth(RD),probing depth(PD),keratinized tissue width(KTW)and clinical attachment level(CAL)were measured and recorded before and 8 months after operation.The mean root coverage(MRC)and complete root coverage(CRC)were calculated.Then t-test was used to evaluate the clinical effect of platelet rich fibrin membrane(PRF membrane)in the treatment of Miller ? gingival recession.Results There was no significant difference in baseline PD,RD and KTW between the test group and the control group(P>0.05),and the RD and KTW of the two groups wereimproved 8 months after operation(P<0.05);There was no significant difference in PD between the two groups in baseline and postoperation(P>0.05),but the mean root coverage of the test group and the control group was(57±20)% and(72±21)%respectively,and the complete root coverage rate was 13.33% and 28.57% respectively,the difference between the two groups was statistically significant(P<0.05).Conclusion Although the root coverage rate of Miller ? gingival recession treated with modified coronally advanced flap+platelet rich fibrin membrane is lower than that of modified coronally advanced flap+subepithelial connective tissue grafts,it can also obtain good root coverage effect,widen the width of keratinized tissue,improve clinical symptoms,and promote early healing of tissue;in addition,due to the application of PRF membrane technology,the second operation area is avoided,the postoperative discomfort is reduced,and the patient's satisfaction is high,which has a certain clinical promotion value.
