Research Purpose:A set of cognitive behavior research program was constructed based on Laswell's "Five W" communication theory,and a 4-minute micro-video was produced(including the propaganda knowledge of kinesiophobia and knee arthroplasty)to preliminarily explore the effect of this program on the cognition and behavior of the patients with kinesiophobia after total knee replacement.The effects of combined online and offline comprehensive intervention methods and offline routine guidance on the degree of kinesiophobia,degree of pain,degree of motion of knee joint,the ability of daily living,and self-efficacy of patients after total knee replacement were compared.To evaluate the value of cognitive-behavioral intervention micropropagation in patients with post-total knee arthroplasty.Research Methods:Review system using literature method,tracking at home and abroad about the kinesiophobia patients in the intervention plan and organizing,through experiment and expert consultancy for early intervention program,using experimental research method,selecting a hospital orthopedic joints between January 2019 and December 2019 in hospital,82 cases of patients with unilateral total knee arthroplasty,parallel,according to the exclusion criteria,select 82 cases who kinesiophobia score >37 points,divided them into control group and intervention group(n = 41).Control group to be offline routine guidance and intervention group shall be combined by the micro transmission way of online cognitive-behavioral intervention,compared two groups of patients with postoperative 1 day,3 days,7 days of TSK score,pain scores,knee flexion of motion(ROM),daily life self-care ability score(ADL)and self-efficacy scores and bedtime for the first time the index changes,and statistical analysis.Research Results:Two groups of patients with general information such as gender(male/female),body mass index(BMI),age,education level(illiteracy/primary school/middle/high school/college degree and above),are engaged in professional(farmers/employees/other),and medical insurance payment method(new farming/town health/at all)the results no difference(P > 0.05),the pain of the patient's disease-related information such as age,surgical position(left/right kneearthroplasties),merge disease(0/1/2/3 and above)results had no difference(P > 0.05).Cognition:there was no difference in TSK score,pain score,and self-efficacy score between the two groups on the first day after surgery(within 24 hours after anesthesia)(P > 0.05).On the3 rd day after the operation,there was no difference in the scores of the two groups(P > 0.05),there was a difference in the scores of self-efficacy(P < 0.05),and there was a more significant difference in the scores of pain(P < 0.01).With the increase of length of stay,the results of the third score(7 days after surgery)of the two groups showed differences in kinesiophobia score(TSK),pain score,and self-efficacy score(P < 0.001).During the hospitalization,TSK and pain scores of the patients showed an overall downward trend,while self-efficacy showed an upward trend.After further repetitive measure analysis of variance,the results showed that the scores on time,group and interaction effect(P < 0.05,two groups of patients with the score in a longer duration of hospitalization after the difference was statistically significant,and the micro transmission intervention method with the result of the will,over time,the influence factors of the effect of changing.Behavior: the results of the intervention group were better than those of the control group(P< 0.05).There was no difference in knee flexural activity(ROM)and ADL between the two groups on the first day after surgery(within 24 hours after anesthesia)(P > 0.05),and there was a difference in the results of the second evaluation on the third and seventh days after surgery(P <0.05).The ROM and ADL scores of the patients in the hospital were on the rise.After repeated measures analysis of variance,the marking result on time,group and interaction effect(P < 0.05,tip ROM and ADL scores of two groups of patients in the longer duration of hospitalization after the difference was statistically significant,and the micro transmission intervention method with the result of the role may change over time,the factors influencing the changes.Research Conclusion:The cognitive-behavioral intervention via micropropagation can effectively reduce the degree of kinesiophobia,reduce the irrational postoperative pain sensation and improve the self-efficacy score of patients during hospitalization after TKA.The cognitive-behavioral intervention via micro transmission can shorten the time for patients to get out of bed for the first time,increase the range of motion of their joints during hospitalization after TKA,improve the ability of patients to take care of themselves,and promoterapid recovery.
