Objectives:To analyze the clinical manifestations,reasonable treatment and prognostic factors of primary thyroid lymphoma(PTL)in order to provide reference for clinical diagnosis and treatment.Methods:The clinical information and follow-up data of 34 patients with PTLtreated in Yunnan Cancer Hospital from January 1,1995 to December 31,2015 were reviewed and analyzed by Kaplan-Meier univariate and Cox regression model.Results:A total of 34 patients with PTL were included in this study,including 24 cases of diffuse large B-cell lymphoma(DLBCL),7 cases of mucosa-associated lymphoid tissue lymphoma(MALT),1 case of follicular lymphoma,1 case of mixed lymphoma and 1 case of T-cell-derived lymphoma.There were 15 males and 19 females,the ratio of male to female was 1:1.275,the age of onset was 22?81 years old,and the median age was 63 years old.The overall 3,5 and 10 year survival rates were 88.2%,75.6%and 58.3%,respectively.The results of univariate analysis showed that gender,serum LDH level at the time of diagnosis of PTL,HT,cTNM stage,pathological classification,operation,chemotherapy and operation combined with chemotherapy affected the prognosis of the patients.Cox regression analysis showed that cTNM stage and chemotherapy were independent prognostic factors of PTL patients.Conclusions:PTL is more common in elderly women,and most of the pathological types are B-cell-derived non-Hodgkin's lymphoma.Male,PTL patients with elevated serum LDH level at diagnosis,late stage with HT and cTNM stage,pathologically classified as highly malignant group/invasive lymphoma,without surgical treatment,without chemotherapy and without combined therapy had a poor prognosis.cTNM stage and chemotherapy are independent prognostic factors of patients with PTL.The treatment should be chosen according to the specific conditions of the patients(age,pathological type,clinical stage,systemic condition,etc.).Comprehensive treatment is more beneficial to improve the prognosis of the patients.Due to the incidence of the disease,the number of cases in this study is relatively small,the above conclusions still need to be further verified.
