Objective(s):Acute myocardial infarction(AMI)is a common,frequently occurring disease,with a high incidence and mortality,which poses a serious threat to human health.The most basic cause of myocardial infarction is the formation of atherosclerosis and the decrease of plaque stability.A large amount of evidence-based medical evidence shows that arteriosclerosis is an effective predictor of mortality from cardiovascular diseases.Atherosclerosis is a systemic,lipid-driven immune inflammatory disease.The most important risk factor for atherosclerosis is dyslipidemia.The classical single blood lipid index can not fully explain and predict the incidence and outcome of cardiovascular diseases.In this study,the atherogenic index of plasma(AIP)?neutrophil-lymphocyte ratio(NLR),and lipid level were compared between the control group(non-myocardial infarction)and the experimental group(myocardial infarction).To evaluate the correlation between plasma arteriosclerosis index and neutrophil-lymphocyte ratio and coronary Gensini score,providing a more convenient and quick clinical basis for active prevention and treatment of early myocardial infarction.To investigate the correlation between plasma arteriosclerosis index.neutrophil-lymphocyte ratio and acute myocardial infarction.To investigate whether plasma arteriosclerosis index and neutrophil-lymphocyte ratio can be used as predictors of acute myocardial infarction.To investigate whether plasma arteriosclerosis index and neutrophil-lymphocyte ratio were different in three subgroups of acute coronary syndrome.Methods:Patients hospitalized in the Department of Cardiology,South Hospital of Gan Mei Hospital,Kunming Medical University from June 2018 to December 2019 with "chest pain" symptoms suspected of acute myocardial infarction received coronary angiography(CAG)or percutaneous coronary intervention.PCI or Percutaneous transluminal angioplasty(PTCA)and assay in 392 patients.According to the diagnostic guidelines,194 patients were divided into non-acute myocardial infarction group and 198 patients into acute myocardial infarction group.For all the object of study records of gender,age,smoking history,history of hypertension,diabetes,on admission heart rate,systolic blood pressure on admission,albumin,serum creatinine,number of number of neutrophils,lymphocytes,plasma total cholesterol,total cholesterol,TC),triglyceride(triglyceride,TG).high-density lipoprotein cholesterol(high density lipoprotein cholesterol,Results of hdL-C,low density lipoprotein cholesterol(LDL-C),homocysteine(HCY),and coronary angiography.The degree of coronary artery stenosis in the two groups was calculated based on the Gensini score.AIP value was calculated according to the formula AIP=log10(TG/HDL-C),and NLR value was calculated according to(neutrophil number/lymphocyte number).Independent sample T test,X2 test score,Shapiro-Wilk statistics,ANOVA analysis,Pearson analysis,curve fitting analysis,Logistic regression analysis to evaluate the predictive ability of AIP and NLR to acute myocardial infarction and calculate its cutoff value.?=0.05 is the test level,P<0.05;The difference was statistically significant(0.05).Results:1.There were differences in smoking,systolic blood pressure,hypertension,creatinine,albumin,HDL-C,LDL-C,neutrophils,NLR,AIP,Gensini scores between acute myocardial infarction group and non-acute myocardial infarction group,P>0.05;The difference was statistically significant.There was no difference in gender,diabetes mellitus,heart rate,age,TG,TC,HCY,lymphocyte,etc,P>0.05;The difference was not statistically significant.AIP,NLR,Gensini score,neutrophils,LDL-C and other indicators were higher in the acute myocardial infarction group than in the non-acute myocardial infarction group,while HDL-C was lower in the non-acute myocardial infarction group.Comparison of basic data among STEMI,NSTEMI,UA and other three subgroups revealed differences in age,systolic blood pressure,combined hypertension,serum creatinine,albumin,HDL-C,LDL-C,neutrophil,AIP,NLR,Gensini scores,P<0.05,especially in neutrophils,NLR,Gensini score and other basic data had significant differences,P<0.01,the difference was statistically significant.2.Pairwise correlation analysis of AIP,NLR,Gensini score showed that the correlation between Gensini score and AIP was not obvious in this study.The Gensini score was positively correlated with the NLR,that is,the higher the NLR value was,the higher the Gensini score was.The correlation between acute myocardial infarction and NLR and AIP was(r=-0.407,P=0.000)and(r=-0.103,P=0.042).respectively.AIP and NLR may be risk factors for acute myocardial infarction,OR(95%CI)were 2.31(1.41-3.8)and 1.79(1.55-2.08),respectively.3.Cutoff values of AIP and NLR with diagnostic value for acute myocardial infarction were 0.97 and 3.27,respectivelyConclusion(s):1.AIP and NLR of patients with acute myocardial infarction were significantly higher than those of patients without acute myocardial infarction;2.AIP and NLR may be risk factors for acute myocardial infarction;3.AIP and NLR have certain value in the diagnosis of acute myocardial infarction.
