Objectives:1.To explore the relationships between femoral cortical thickness index(CTI)and femoral neck bone density,to verify the feasibility and effectiveness of predicting osteoporosis with femoral CTI,so as to quickly obtain a diagnosis of osteoporosis in the clinical practice;2.To assess the correlation between femoral CTI and osteoporotic fracture risk and the reliability of predicting osteoporotic fracture risk with femoral CTI,providing a simple,rapid and quantifiable method for further clinical decision-making and treatment;3.To explore the effects of age and gender on the outer diameter of the femoral shaft(DW),the intramedullary width(IW),and the thickness of the cortex(DW-IW)of the femur in order to provide a preliminary basis for further research on the environmental changes of the femoral bone marrow cavity and the pathogenesis of osteoporosis.Methods:This subject uses the retrospective analysis method to collect 280 inpatients and outpatients from June 2018 to September 2019.According to the inclusion and exclusion criteria,128 cases were excluded,and 152 subjects were included,which aged between 40 and 90 years old,there were 81 females and 71 males.These subjects all have been take X-ray orthophoto and femoral neck bone mineral density scan on the same side femur.Measure IW?DW-IW?CTI on the X-ray of the femur and use the fracture risk assessment tool(FRAX)to calculate the corresponding fracture risk within 10 years according to the patient's medical history and related information,then analyze its correlations with neck bone mineral density and fracture risk status,assess the ability of CTI to predict osteoporois satus and osteo orotic frapcture risk status,then calculate the threshold.Results:There were significant differences in DW?IW?DW-1W?femoral neck bone mineral density and fracture risk status were observed between males and females.Femoral CTI was significantly positively correlated with femoral neck bone mineral density in both gender.and negatively correlated with fracture risk status(p<0.05),and femoral CTI was negatively correlated with age(p<0.05).The femoral CTI has a good effect to predict osteoporosis status and fracture risk status.The prediction thresholds of osteoporosis status for males and females are:?0.54 and?0.60;The prediction thresholds of high risk of major osteoporotic fractures and hip fractures aremales:?0,53 and ? 0.57,females:? 0.51 and ? 0.56:The widening of the femoral bone marrow cavity is positively correlated with age,cortical thickness is negatively correlated with age,and age affects females more than males;there is no correlation between outer diameter of femoral shaft and age.Conclusions:1.Femoral CTI is significantly positively correlated with femoral neck bone mineral density;Femoral CTI has good prediction ability of osteoporosis;Femoral CTI?0.54(males)and?0.60(females)can be used as the threshold for osteoporosis diagnosis;2.In clinical practice,besides bone mineral density and FRAX,femoral CTI is also reliable as an auxiliary tool for fracture risk status evaluation;The prediction thresholds are males:?0.53 and?0.57,females:?0.51 and?0.56 when use femoral CTI to predict high risk of major sites fracture risk and high risk of hip fracture risk;3.With the increase of age,there is no obvious change in the outer diameter of femoral shaft;the femoral bone marrow cavity is widened and the cortex is thinned.These changes are more obvious in women than men,and it is mainly caused by the absorption of inside cortex of the femur.This research provided a preliminary basis for further research on the environmental changes of the femoral bone marrow cavity and the pathogenesis of osteoporosis.
