PART ? Study on the nutritional status and prognostic value of patients with AIDS-related lymphomaObjective:Retrospectively analyze and explore the nutritional status of patients with AIDS-related lymphoma and its value in prognosis.Methods:Collected data on 69 patients with AIDS-realted lymphoma admitted to Yunnan Infectious Diseases Hospital/Yunnan AIDS Care Center from January 2017 to January 2019.According to the prognosis outcome,they were divided into survival group and death group for exploring its clinical characteristics and prognosis.Results:Among 68 patients with AIDS and lymphoma,57 were male(82.61%)and 12 were female(17.39%),the sex ratio was 4.75:1;The median age was 48 years(21 to 73 years).HIV transmission was mainly sexually transmitted,HIV and lymphoma were found in 35 patients at the same time,and the average time to develop ARL from HIV infection in 34 patients was 53.03 months,with a maximum of 168months;A total of 20 patients(28.99%)with BMI<18.5Kg/m2,a total of 38 patients(55.07%)with DPI scores of high to high risk,and 31 patients(44.93%)with PA<170mg/L;The one-year survival rate was 78.26%.Univariate analysis showed that the IPI score,BMI,ALB,PA,and CD4T lymphocyte count were related to the clinical outcome.Regression analysis was performed by COX risk ratio.IPI score,BMI,PA were found to be risk factors influencing the survival rate of AIDS-related lymphoma.Further,by drawing the ROC curve and comparing the area under the curve,it was found that the composite indicators of IPI,BMI,PA are better than single indicators in predicting clinical outcomes.Conclusion:Most patients with AIDS-related lymphoma are in advanced disease at the time of diagnosis,and malnutrition is common.Nutritional status has important value in the development of AIDS-related lymphoma;BMI and PA combined with IPI score can better screen high-risk patients and carry out risk stratification.PART ? Study on the influence of nutritional support on nutritional status of AIDS-related lymphoma patientsObjective:To study the nutritional status of patients with AIDS-related lymphoma before and after nutritional support.Provide theoretical basis for clinical nutrition treatment.Methods:Collected 15 cases of AIDS-related lymphoma patients who met the research requirements from the Yunnan Infectious Diseases Hospital/Yunnan AIDS Care Center since November 1,2019,and received nutrition education and personalized nutrition support upon admission.Fifteen patients were randomly selected from the previous study as a control group.To study BMI,PA,ALB,CD4 lymphocyte count and IPI score changes and statistical analysis.Results:The BMI and PA values of patients were affected by nutritional intervention and time interaction.With time,the BMI and PA values of the control group showed a downward trend,and the BMI and PA values of the experimental group receiving nutrition support showed an upward trend with time;There was no significant difference in BMI and PA values at the time of admission;there was no significant difference in BMI values between the experimental group and the control group after 1 month of treatment,and the difference in PA values was statistically significant;2 months after the treatment,the BMI,The differences in PA values were statistically significant.There was no statistically significant difference in ALB and IPI score changes between the two groups of patients;CD4 increased in both groups,but the difference was only statistically significant between the experimental group admission and after 1 month and 2 months of treatment.Conclusion:Individualized nutrition support can effectively improve the nutritional status of patients,increase and maintain patient weight to a certain extent,and increase PA value.Nutritional intervention cannot cause significant changes in ALB in a short period of time,it is difficult to reverse the development of the disease and play a role in degrading the IPI score,but it may help the patient to reconstruct the immune deficiency.
