OBJECTIVEIn this prospective study, the accuracy of computer assisted real-time navigation technology in dental implant surgery was calculated and compared between the maxillary and mandibular, anterior and posterior regions, flap and flapless. The factors influencing the accuracy of computer-aided real-time navigation technology and the method of improving accuracy were discussed and analyzed, which provided theoretical basis for further improving the accuracy of implant clinical prosthesis.METHODSAccording to the inclusion criteria,we enrolled a sample composed of patients who had computer assisted real-time navigation implants placed between June 2017 through December 2018 in Yantai Stomatological Hospital. The cone beam scan was taken and the intraoral scan was performed, then transfer the digital information to the navigation system's computer.Virtual implants were placed and oriented in a position that allowed for the planned restorative care. Then the implants were placed in aid ofthe computer assisted real-time navigation software and the post-surgical CBCT was performed. The errors between the designed and the actually placed implants were measured to evaluate the accuracy and effectiveness. The pre-op virtual implant and the post-op actual implant were displayed in the same three-dimensional coordinate system. The errors of shoulder distance, root distance and angle of each implant were measuredThe data was analysed by SPSS 22.0 software. The data in each group were expressed as mean±standard error(X±s).Independent sample t test was used to compare the measurements among the groups. The test level was a=0.05,P<0.05 was considered to have significant difference.RESULTS1.The average erorr of shoulder distance, root distance and angle was 0.61±0.17mm(0.35?0.98mm),0.73±0.15mm(0.50?0.99mm)and(3.32±1.55)°(0.43°?6.66°)respectively.2.Comparing the accuracy of computer-assisted real-time navigation implantation in maxillary and mandibular, the shoulder distance error was 0.59±0.16 mm and 0.63±0.19 mm,the root distance error was 0.75±0.14 mm and 0.70±0.18 mm,and the angle error was(3.46±1.80)° and(3.07±1.12)° respectively.3.Comparing the accuracy of computer-assisted real-time navigation implantation in anterior and posterior regions, the shoulder distance errOr was 0.60±0.21mm and 0.60±0.16mm,the root distance error was 0.72±0.16mm and 0.72±0.15mm,and the angle error was(3.28±1.85)°and(328±131)°respectively.4.Comparing the accuracy of computer-assisted real-time navigation implantation in flap and flapless, the shoulder distance error was 0.68±0.17mm and 0.55±0.15mm.the root distance error was 0.81±4.15mm and 0.67±0.12mm,and the angle error was(329±1.65)° and(3.35±1.58)°respectively.CONCLUSION1.Computer assistred real-time navigation in dental implant surgery exist errors, the errors are within the allowable range. It can be widely used in preoperative implant assessment, intraoperative operation guidance,post-operative accuracy analysis.2.The accuracy of computer-aided dynamic navigation system have no obvious deviation in shoulder distance, root distance and angle. In clinical application, reasonable selection should be made according to indications.
