Objective:To statistically analyze the disease spectrum of hypersensitive cardiac troponin T(hs-cTnT)inpatients in the affiliated hospital of North Sichuan Medical College to provide retrospective data reference for clinical work.Method:Retrospectively collected 1376 patients who were tested for hs-cTnT during the hospitalization period from September 2018 to December 2018 in the Affiliated Hospital of North Sichuan Medical College,and recorded their basic information,laboratory data,diagnosis,and hospital outcomes.The characteristics of the disease spectrum of hs-cTnT-positive hospitalized patients were compared.The disease spectrum of different sex,age,renal function,and elevated levels of hs-cTnT was compared,and the relationship between death and related risk factors was analyzed using binary logistic regression.Results:1.A total of 1376 hs-cTnT-positive patients were included in the study.Among them,there were more male patients than females(gender ratio 1.646),with an average age of 67.6±16.291 years.The 10 most common diseases of hs-cTnT-positive patients were hypertension and pulmonary infection,peripheral arterial disease,electrolyte disturbance,stable coronary heart disease,heart failure,anemia,renal insufficiency,hypoalbuminemia,diabetes;acute coronary syndrome(ACS)accounts for 13.01%,non-ACS patients 86.99%;compared with non-ACS patients,ACS patients have a higher proportion of males and are older.Cardiovascular medicine,nephrology,respiratory medicine,neurology,and geriatrics are the departments with the highest number of hs-cTnT positive patients.2.Divided into male and female groups according to gender,of which 856 were males(62.20%),with an average age of 68.35±15.544 years;520 females(37.80%),with an average age of 66.36±17.396 years,the average age of males was greater than females;compared with females,males had higher prevalence of various ACS.Among the non-ACS diseases with the highest prevalence,males had higher rates of peripheral arterial disease and chronic obstructive pulmonary disease(COPD)than females,and females had higher rates of hypertension,pulmonary infection,electrolyte disturbances,anemia,renal insufficiency,and heart failure than males.The prevalence of stable coronary heart disease is equal for males and females.3.Grouped by age,among the most common diseases of hs-cTnT positive patients,pulmonary infection and hypertension are common in patients of all ages,renal insufficiency and anemia are relatively common in young and middle-aged patients,peripheral arterial disease,stable coronary heart disease,COPD,and cerebral infarction are relatively common in the-young-old and the-old-old patients;the prevalence of cardiovascular diseases such as ACS is increasing with age,but ACS reaches the highest among the-young-old patients,and Slight decline in the-old-old patients.4.Grouped by renal function,with the reduction of glomerular filtration rate,the prevalence of hypertension,anemia,renal bone disease,and heart failure has increased.However,in extremely severe renal insufficiency,the hemodialysis patients increased,but the prevalence of heart failure decreased.Pulmonary infections and hypertension were most common in each renal function group.5.According to the hs-cTnT level,the results showed that the highest prevalence rates of several diseases were similar:hypertension,pulmonary infection,peripheral arterial disease,electrolyte disturbance,stable coronary heart disease and renal insufficiency.With the increase of hs-cTnT level,the prevalence of ACS and renal insufficiency increased,and the prevalence of MI increased significantly in patients with severe increase.The prevalence of UA was similar in all hs-cTnT groups.6.Among patients with hs-cTnT positive,the mortality of ACS patients was 6.15%,the mortality of non-ACS patients was 4.59%,and the mortality of ACS patients was higher than that of non-ACS patients(P<0.05).Increased of hs-cTnT and N-terminal B-type natriuretic peptide(NT-proBNP)can increase the risk of death for patients.Conclusions:Both ACS and non-ACS diseases may lead to increased levels of hs-cTnT;patients with elevated hs-cTnT have different disease spectrum characteristics in different department,genders,ages,renal functions,and elevated levels of hs-cTnT;increased hs-cTnT and NT-proBNP levels can increase the risk of death for patients.
